| Objective Dixon technology was used to measure the fat content of lumbar vertebrae,and to evaluate the value of lumbar osteoporosis,and to explore on patients with lumbar endplate inflammation and glucocorticoid therapy in patients with rheumatoid vertebral body fat content and the relationship between bone mass.Method1.144 of 36 cases of patients with vertebral bodies are studied,for different DXA bone mass group MR signal intensity of the lumbar spine(T2 lipid phase)and fat fraction(FF)analysis;2.study Of 48 patients with MR images hint of inflammation of lumbar vertebral endplate and normal without the endplate infection control group,Comparative analysis of DXA measurement waist 1-4 vertebral bone mineral density(BMD)values(g/cm2)and MR image measurement FF;3.Comparative analysis of different age groups(30-40 years,40-50 years old)20 cases in each group of rheumatoid patients,before treatment and 3 months after treatment and 6months after fat fraction(FF)and lumbar spine BMD values.Result1.According to the results of DXA were divided into three groups of normal bone mass group,bone mass reduction group,osteoporosis group,the three groups of lumbar T2 WI lipid phase signal strength and fat scores were statistically significant(P<0.01).There was a significant negative correlation between the signal intensity and fat fraction of DXA;2.There was a significant difference in BMD value between the patients with endplate inflammation and the normal control group(P<0.001);There was no significant difference inBMD value of each type of endplate inflammation group;The FF value of the endplate inflammation group was significantly higher than that of the normal control group(P<0.001);Comparison between groups of endplate inflammation,Modic I and Modic II and Modic III were statistically different(P<0.001);There was no statistical difference between ModicⅡand ModicⅢ(P=0.54);3.There was a significant difference in BMD value between the age groups of patients with rheumatoid disease before treatment(P=0.007),All patients had significant correlation with FF and BMD.After 3 months of treatment,the level of fat score was significantly higher than that of the medication group for 6 months,and the increase of 40-50 years old age group was higher than that of 30-40 years old age group.Conclusion1.Magnetic resonance(NMR)water and fat separation imaging can reflect the change of the fat content of the vertebral body by measuring Lumbar signal intensity and calculating the fat fraction,to make a preliminary diagnosis of osteoporosis;It has a certain application prospect in the evaluation of lumbar bone mineral density;2.Magnetic resonance(NMR)water and fat separation imaging as a non-invasive measurement on the basis of routine MR examination method of vertebral bone marrow fat content,can be found much earlier than DXA bone density measurement inflammation in patients with lumbar endplate changes of cancellous bone strength;3.In the process of Different types evolution of endplate inflammation may be accompanied by the change of the vertebral strength;4.Long term use of hormone therapy in patients with rheumatoid arthritis due to increased vertebral body fat content,bone mass decreased resulting in decreased bone strength,and with the increase of age and medication time,bone strength decreased more obviously. |
