Relationship Study Between Serum FGF-21,Adropin Level And Diabetic Kidney Disease

Objective: To observe the levels of serum fibroblast growth factor 21(FGF-21)and Adropin in type 2 diabetes mellitus and diabetic kidney disease individuals,and to investigate the relationship between FGF-21,Adropin and diabetic kidney disease.Methods: 114 patients with type 2 diabetes mellitus were divided into type 2 diabetes mellitus group(T2DM),micro albuminuria group(mic-DKD)and macro albuminuria group(mac-DKD)according to urinary albumin excretion rate(AER).Meanwhile,36 patients with impaired glucose tolerance were selected as impaired glucose tolerance group(IGT),and 42 healthy people were selected as normal control group.Fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL-C),low density lipoprotein(LDL-C),serum creatinine(SCr),fasting insulin(FIns),2 h postprandial insulin(2hINS)level and AER were determined.The data of height,weight,waist circumference(WC),systolic blood pressure(SBP)and diastolic blood pressure(DBP)were selected.Body mass index(BMI),glomerular filtration rate(eGFR),urinary albumin to creatinine ratio(ACR),insulin resistance index(HOMA-IR)and insulin secretion index(HOMA-?)were calculated.Enzyme linked immunosorbent assay(ELISA)was used to determine the levels of serum FGF-21 and Adropin.Results:1.The levels of WC,SBP,DBP,FPG,HbA1 c,AER,HOMA-IR,ACR,FGF21 in micro albuminuria group and macro albuminuria were significantly higher than those in normal control group.(P<0.05).The levels of HDL-C,HOMA-?,eGFR,Adropin in type 2diabetes mellitus group,micro albuminuria group and macro albuminuria group were significantly lower than that of normal control group(P<0.05).2.Adropin was negatively correlated with SBP,DBP,FPG,HbA1 c,TG,AER,ACR and FGF21(P<0.05).3.FPG,HbA1 c and Adropin were independent risk factors of AER in diabetic kidney disease patients.Conclusion:FGF21 may be associated with diabetic kidney disease,but it is unclear that something affect occurrence and development of diabetic kidney disease.Low levels of Adropin may be involved in the development of diabetic kidney disease.
