The Comparative Study Of The Short-term Curative Effect Of Cast Porcelain Onlay And Co-Cr Porcelain Crown Restoration In Root Canal Therapy Of Posterior Teeth

ObjectiveThrough comparing the short-term curative effect of two different repair technique after root canal treatment of molar teeth,namely cast porcelain onlay repair and cobalt chromium porcelain crown repair,the study evaluated the curative effect after 1 year of treatment of four aspects including the integrity of the restoration contour,periodontal health,food impaction,restoration margins,so as to provide theoretical basis for the clinical choice of repair technique after root canal therapy in molar teeth.Subjects and methodsCollect 120 cases of pulpitis patients caused by tooth decay and need to do a root canal surgery which were treated at the dental clinic of affiliate hospital of logistics university of PAP during July 2014 to December 2015 to enter the study group.These patients are aged from 22 to 69(average 35.9),including 68 molars,52 premolar,58 maxillary,62 mandibular teeth.According to the random distribution of meter,120 cases of patients were separated into 2 groups to perform 2 different repair treatment one week after root canal treatment and periodontal basic treatment,the all-ceramic onlay group of 60 cases,32 cases of male,female 28 cases,while cobalt chromium cobalt chromium porcelain crown group of 60 cases,the male 34 cases,female 28 cases.The short term clinical effect of these two repair methods were compared respectively from a total of four aspects,namely the integrity of the restoration contour,periodontal health,food impaction and restoration margins according to the 3 months,6 months after treatment,12 months follow-up visit results.The evaluation index of four aspects is respectively:(1)the integrity of the restoration contour include: “A” represents the restoration integrity is good,“B” represents the restoration has a small defect in the appearance but does not affect the usage,“C” represents he restoration has broken,“D” represents the restoration fall off.(2)the periodontal health include: “A” represents periodontal has no inflammatory,“B” represents mild periodontal inflammation and “C” represents periodontal inflammation,bleeding gums,and periodontal pocket can be detected(3)food impaction includes: “A” represents adjacency relations with adjacent teeth is good with no food impaction,“B” represents the adjacency relations with adjacent teeth is loose with food impaction.(4)restoration margins including: “A” represents edge seal is good,clearance is not able to be detected by a probe,“B” represents clearance is able to be detected but can't go into by a probe,“C” represents clearance is able to be detected and can go into by a probe.ResultsThe index of four aspects of all-ceramic onlay repair method and cobalt chromium porcelain crown repair method were compared.there were no statistical significance(P > 0.05)in integrity of the restoration contour,food impaction and restoration margins,while in periodontal health,the evaluation index showed statistically significant(P < 0.05).In terms of the integrity of the restoration contour,the broken rate is 5% in both all-ceramic onlay group and cobalt chromium porcelain crown group showed by 12 months follow-up visit results.In terms of the Fall off rate,the all-ceramic onlay group had a rate of 7%,while the cobalt chromium porcelain crown group showed a rate of 13%.In terms of periodontal health,all cases of the all-ceramic onlay group had healthy periodontal,at a rate of 100%,while cobalt-chromium cobalt chromium porcelain crown group with no inflammation was only 82% showed by 12 months follow-up visit results.Conclusion1.The short term curative effect of all-ceramic onlay repair method and cobalt chromium porcelain crown repair method were almost the same at three aspects including the integrity of the restoration contou,the integrity of the food impaction and restoration margins,while the all-ceramic onlay methods is distinctly better than co-cr cobalt chromium porcelain crown at periodontal health2.In terms of the integrity of the restoration contour,cobalt-chromium cobalt chromium porcelain crown is easier to fall off compared with higher than all-ceramic onlay.
