| Objective Somatoform disorders(SFD),a common psychic disorder.The patient with this disorder always seek treatment repeatedly because of physical symptoms.The patient's doubt about being sick can not be easily eliminated for all kinds of normal medical examination results and repeated confirmation from his physician.The first diagnose of this disorder often been made in the general hospital where the rate of identification of this disorder was low and the anxiety and depression of patient were easily to ignored by the non-psychiatrists.Patient satisfaction and treatment compliance were reduced for the incorrect diagnose and fail sympathize with the disease stress.At the same time,the incidence of medical disputes and patient's own suffering as well as economic burden were increased.This article aims to explore the mental health status of SFD patients in general hospital and the relationship between the development of the disease and psychological factors through comprehensive research and analysis so as to enable non-psychiatrists attach more importance to SFD and provide basis for complex treatment with this disorder.Method 120 patient which meet the ICD-10 diagnostic criteria for Somatic form Disorders presented between 2015-2016 in Tianjin Port Hospital were taken in the observation group,and 120 subjects volunteers were recruited as healthy control group.SCL-90,EPQ,DSQ,SSRS were used in both group respectively.Compare mental health status and psychological traits in two group and analysis the correlation between mental health status and psychological traits in the observation group.All statistical analysis was performed using SPSS17.0 statistical software,P<0.05 was set as statistically significance.Enumeration data were compared using chi square test and measurement data were compared using T test and analysis of variance.Logistic regression analysis was conducted with SCL-90 score whether higher than 160 as a dependent variable,and the significant factors in single factor analysis as independent variables.Result The results show that in mental health status,the score of The SCL-90,somatization,anxiety,depression,paranoid in the observation group were higher than those in the control group and the difference was significant(P<0.05),differences had statistical significance(P<0.05).The patients in the observation group are always introversion personality.Neurotic score of the observation group was higher than the control group and the difference was significant(P<0.05).In terms of defense,immature defense mechanism score of the observation group was higher than the control group,while maturity defense mechanism score of the observation group was lower than the control group and the differences were significant(P<0.05).In terms of life event,the total score of the life events and negative events was higher than the control group and the difference was significant(P<0.05).In social support,the scores of objective social support,subjective social support and social support utilization were significantly lower than the control group(P<0.05).Psychosocial factors are different among SD ? USD and SFAD.Mental health was negative correlated with nervosity,immature defense mechanism and positive correlated with social support,and had a regression relationship with age,gender,education,nervosity value,in patients with somaticform disorder.Conclusion Much attention should be attached to the correct diagnose and formal treatment of patients with SFD in general hospital since they have a tendency of having somatization,anxiety,depression and other issues.Most of the patients with SFD were emotionally unstable.Defense style and social support are closely related to the development of SFD.All these results can be incorporated into the category of psychotherapy intervention,and comprehensive treatment for SFD patients should be performed. |
