ObjectiveThrough the study of preoperative NLR and serum TSH levels to understand the differences between the two indicators in patients with thyroid nodules,and to explore the clinical application value of NLR and serum TSH as a preoperative diagnostic thyroid nodule benign and malignant index.MethodsRetrospective analysis of 327 cases of basic surgery in Fujian Provincial Hospital of surgical treatment of thyroid nodules from January 2015 to December 2015 in patients with clinical data.All patients were primary tumor in the thyroid,have a complete clinical history data,within 1 month before the urinary system,digestive system,respiratory system and other effects of blood infection.Patients were collected with age,sex,serum TSH level,neutrophil count lymphocyte count,tumor size,invasion of thyroid capsule,tumor properties,central lymph node metastasis,other information and calculate the NLR value.The collected data were analyzed by SPSS19.0,the diagnostic results of preoperative NLR and serum TSH were analyzed by the receiver operating curve.ResultsIn the sex composition of thyroid benign nodules and thyroid malignant nodules,female patients were more likely than men.Thyroid benign nodules group NLR lower than the thyroid malignant nodules group,the difference was statistically significant(P<0.01).The level of NLR in thyroid nodule group was higher than that in thyroid nodule group(P<0.01).The level of serum TSH in thyroid benign nodule group was lower than that in thyroid malignant nodules group(P<0.01),with the increase of serum TSH level,the malignant rate of thyroid nodule was increasing.Small thyroid nodules group serum TSH level is higher than large thyroid nodules,statistically significant difference(P<0.01).The optimal cut-off values of NLR,serum TSH level and NLR combined with serum TSH level in guiding thyroid nodules were 1.89mIU/L,1.88 and 0.55 in ROC,and the sensitivity was 61.8%,55.6% and 69.7%,the specificity was 73%,86.6% and 79.9%,the area under the curve was 0.689,0.718 and 0.793.Conclusions1.Thyroid benign nodules of NLR,TSH lower than PTC.2.PTC tumor staging,capsule invasion and lymph node metastasis in central region has nothing to do with the NLR and TSH.3.NLR combined with serum TSH levels on the diagnosis of thyroid nodules have a better effect.
