Systematic Review: The Clinical Significance Of Probable Positive During Treadmill Exercise Test In Chinese People

Object: The treadmill exercise test is widely used in noninvasive screening and following-up of coronary artery disease because of its sensitivity,safety and relatively low price and convenient management.There are many confuses about the result of probable positive which is between positive and negative.The purpose of this paper is reviewing a series of study about the probable positive result of TET systematically and further clarifying its clinical significance on noninvasive screening of suspected coronary artery disease by Meta-analysis.Methods: Search related articles in the database and screen the articles according to inclusion and exclusion criteria.Extract the data which we need from the selected articles.The data were analyzed by Meta-analysis in the statistical software,Open-Meta[Analyst] and Review Manager 5.3.Result: The sensitive of probable positive in TET is 31.0% ~ 48.4%,43.5% ~57.6% for male and 10.0%~29.9% for female.The ratio difference between male and female is 27%~43%.This difference has statistically significant.The ratios of coronary artery multivessel stenosis in the patients of coronary artery disease whose result of TET is probable positive is 22.8%~35.4%,and there are not difference between sexuality.For the patients who TET report probable positive but doesn't have coronary artery disease,the constituent ratio of autonomic dysfunction,hypertension and diabetes is43.6%~60.7%(8.4%~32.6% for male,15.1%~51.9% for female),22.4%~32.9%(51.2%~90.3% for male,6.7%~32.6% for female),3.3%~11.9%,respectively,and there are not sexuality differences in diabetes.Conclusions: The sensitivity of probable positive in TET for the diagnosis of coronary artery disease is not so good.Those patients who TET probable positive and have coronary artery disease are more likely to be single vessel disease.But for men,the diagnostic value of TET was significantly better than that of female.The sensitive of probable positive in male may be no less than positive.The primary diagnosis of those patients who TET probable positive but don't have coronary artery disease is most likely to be autonomic dysfunction,but there are mainly female.It's more likely to be hypertension for male.It is because there more female in those patients.Abnormal vascular function,normal blood vessels couldn't meet the needs of perfusion in the reconstructed myocardium may be the reason of false positive.
