Objective:In this study,we investigated the serum levels of insulin resistance(insulin resistance,IR)and tumor necrosis factor-?(tumor necrosis factor-?,TNF-?)in patients with recurrent aphthous ulcers(RAU)and obese(OB),RAU or OB alone and healthy people to analyze the effect of RAU to IR on obese individuals and the correlation with TNF-?.To guide RAU patients with early screening,diagnosis and intervention of IR The incidence of pre-diabetes and delay its conversion to diabetes to provide a reference.Methods:In this study,Forty patients were randomly selected from November 2015 to December 2016 at the First Hospital of Shanxi Medical University and were diagnosed with light RAU.According to the body mass index,RAU compound obesity group(20cases)and RAU group(20 cases)and control group(20 cases)were randomly divided into obese group(20 cases)and control group(20 cases)according to the body mass index.Four groups were used in the United States Beckman Coulter produced AU5821 series of automatic biochemical analyzer using carbohydrate kinase method to detect serum fasting blood glucose levels,Using the GC-1200 ? radioimmunoassay produced by China Science and Technology Innovation limited company to detect the levels of fasting insulin in serum were measured by radioimmunoassay.The insulin resistance index(HOMA-IR)was calculated by the formula.The continuous-wavelength microplate reader produced by American Molecular Instruments was used to detect TNF-? content.By the same dentist to patients with oral mucosal examination and record the location of RAU attack,size and number.The four sets of personal data collected include age,gender,height,weight and past medical history,as well as the measured data recorded in the table.SPSS22.0statistical analysis software was used to analyze the experimental data,and the results were statistically different when P <0.05.Results:1.The TNF-? levels in RAU + OB group were higher than those of RAU group and OB group(P <0.05,P <0.05).RAU group and OB group were higher than those in control group(P <0.05,P <0.05).RAU and OB could affect the TNF-? content(P <0.05,P <0.05),and they had synergistic effect(P <0.05).2.The serum fasting blood glucose concentration in RAU + OB group was higher than that in RAU group(P <0.05).RAU + OB group was higher than that in OB group(P>0.05).RAU group and OB group were higher than those in control group(P> 0.05,P>0.05).RAU could affect the serum fasting blood glucose concentration(P <0.05),and the effect of OB was not obvious(P> 0.05),and there was no synergistic effect(P> 0.05).3.The serum fasting insulin concentration in RAU + OB group was higher than that in RAU group and OB group(P <0.05,P <0.05).RAU group and OB group were higher than those in control group(P <0.05,P <0.05).RAU and OB could affect serum fasting insulin concentration(P <0.05,P <0.05),but there was no synergistic effect(P> 0.05).4.The insulin resistance index(HOMA-IR)in RAU + OB group was higher than that in RAU group and OB group(P <0.05,P <0.05).RAU group and OB group were higher than those in control group(P <0.05,P <0.05).RAU and OB could affect insulin resistance index(P <0.05,P <0.05),but there was no synergistic effect(P> 0.05).5.Correlation analysis showed that there was a positive correlation between serum TNF-? and HOMA-IR(P <0.05).Conclusions:1.Patients with active RAU may exist serum TNF-? and IR levels,and the effect of obesity is more significant.2.Patients with active RAU may exist an increase in fasting blood glucose levels.3.The expression of TNF-? in serum was positively correlated with insulin resistance index.
