Study On The Status Of Centralized Tender Of Drug And Influencing Factors Of Drug Distribution In Basic Medical Institutions In Shanxi Province

Objective:By comparing drug distribution in basic medical institutions of Shanxi Province before and after centralized tender of essential medicine in 2015,the paper reflects the status of centralized tender of drug,and analyzes the influencing factors of drug distribution for basic medical institutions in Shanxi Province.The paper puts forward an effective way to improve the distribution rate of essential medicine for basic medical institutions,and provides theoretical foundation for building thorough system of centralized drug tender.Methods:With method of literature analysis,statistical analysis,the paper analyzes creatively drug distribution rate for basic medical institutions and other factors in process of drug centralized tender.It makes statistics and analysis of data of essential medicine tender of basic medical institutions from 2015-2016,Shanxi Province and double envelope index of essential and low-priced drug centralized tender in 2015,Shanxi Province.Using drug distribution rate as an important index of drug supply,the result reveals that policy of drug centralized tender and some special indexes in the process of drug centralized tender do have impact on distribution rate of drugs.Results:Due to the adoption of improved two-envelope selective tender system in process of essential drug centralized tender in 2015,Shanxi Province,the number of shortlisted essential medicine in centralized tender increased,the choice range of essential drug for basic medical institutions expanded magnificently.The price of all of shortlisted essential medicine are lowest.The average score of all of shortlisted essential medicine is 83.89,and it is higher than 81.12,which the average score of all essential medicine in centralized tender.The average score of all following shortlisted essential medicine is 85.22,and it is higher than 82.48,which the average score of all lowest price essential medicine in centralized tender.The distribution rate of essential medicine of basic medical institutions of Shanxi Province in 2016 reaches 94.94%,making an increase significantly compared with 83.50% in 2015.So it can meet basically the need of clinical application of drugs for basic medical institutions.The number of drugs in shortage in basic medical institutions of Shanxi Province in 2016 is 19,making a decrease significantly compared with 175 in 2015,most of shortage problems of drugs was resolved.There is the positive correlation between distribution rate and drugs price(r =0.413,P<0.001),drugs price level(r =0.255,P<0.001),amount of drug purchases(r =0.343,P<0.001),sales of drug purchase(r =0.600,P<0.001)of basic medical institutions of Shanxi Province in 2016.Conclusions:The adoption of improved two-envelope selective tender system in process of essential drug centralized tender in 2015 is scientific and reasonable,it effectively picked out the high quality medicine with the lowest price as shortlisted essential medicine,and it solved the shortage problem.Drug profits is the most important factor to impact the distribution rate.We should lead the enterprise improve the distribution rate of low price drugs or less purchase drugs while they gain reasonable profits from drugs with high profit,then promote the healthy development of the drug supply market.We should put focus on the distribution of low-priced drugs for fear that shortage of these low-priced drugs affects the clinical application of drugs for basic medical institutions.
