Effect Of Peripheral Blood Absolute Lymphocyte Count On The Prognosis Of Newly Diagnosed Patients With Multiple Myeloma

Objective:To explore the correlation between peripheral blood absolute Lymphocyte count(ALC)and the clinical features and prognosis of patients with newly diagnosed Multiple myeloma(MM).Methods:A retrospective analysis of 78 cases of newly diagnosed MM patients from January 2005 to December 2014,we divided patients into two groups according to admission ALC which was less than 1.315×109/L(n=58)and over 1.315×109/L(n=20)to analysis the relationship between ALC and clinical features,therapeutic effect and survival situation.The statistical analysis was performed applying SPSS.the median(range),chi-square test,Kplpan-Meier method,Log-rank test data,Cox regression model for data.Results:Among the 78 newly diagnosed MM patients,45 were male and 33 were female,and ratio between them was 1.36:1.The median age was 65 years(41-87 years),and the median survival time was 19.10 months.According to the onset ALC of MM patients,taking 1.315 ×109/L as cut-off point according to the ROC curve,ALC<1.315×109/L group was often accompanied by low white blood cell count(p< 0.001),low platelet(p=0.03),calcium(p<0.001)and high DS stage(p=0.001),but there were no significant differences between two groups in gender,age,tumor type,ISSstaging,hemoglobin,serum albumin,globulin and the effect of treatment(p > 0.05).Single factor analysis showed that the high age of onset(p=0.017)and low ALC(p=0.046)were the adverse factors of overall survival.Multivariate analysis showed that high age of onset(HR=1.041,95%CI 1.010-1.073,p=0.01)and low ALC(HR=0.335,95%CI 0.157-0.786,p=0.008)were independent prognostic risk factors for MM patients.Conclusion:1.Multiple myeloma is more common in male patients over 65 years old and the main clinical manifestations are bone pain and anemia.Most of the patients are at the late stage of treatment.2.The survival time of the patients with multiple myeloma being low lymphocyte group is low.3.The low lymphocyte count and age of more than 65 years old are independent risk prognosis factors of multiple myeloma.
