A Study On The Interests Of Stakeholders In Public Hospital Reforms

Background With the deepening of the reform of medical and health system in our country,the reform of public hospitals has also shifted from local pilot to comprehensive.However,there are many stakeholders in the process of public hospital reform,and there are different interests in the main body and stakeholder Appeal,and the existence of potential conflicts of interest between the demands of poor coordination will hinder the smooth progress of public hospital reform.Objective Through a large number of access to literature and expert advice to determine the current stage of public hospital reform process stakeholders.Through the questionnaire,to quantify the interests of public hospitals to reform the interests of stakeholders,for the smooth progress of public hospital reform to provide a basis for reference.Methods Based on the previous researchers' research on the stakeholders of public hospitals,the stakeholder maps were drawn and the stakeholders of public hospitals were reformed by questionnaire.Through the analysis of the results of the survey of interest,the principal component analysis was carried out to extract 3-4 factors.After the values are assigned,the factor mean is calculated to sort the factors,and the statistical significance of the sorting of the "paired sample T test" between the difference between the two variables is given.Results 1.The scores of hospital staff,patients and their families,health administrative organs,urban workers(residents)medical insurance institutions and new rural cooperative medical institutions were more than 4.50,which were significantly higher than those of other stakeholders.2.The factor analysis of patients' interests claims extracted three factors,their mean values were 4.458,4.521,4.282,P <0.01 for paired samples,and the mean difference was statistically significant.Through the difference analysis and single factor analysis of variance,it was found that the mean number of factor 3 increased with age,and the difference of factor 1 and factor 3 was different.3.The factor analysis of hospital staff's interests claims extracted three factors,their mean values were 4.286?4.039?4.406,P <0.01 for paired samples,and the mean difference was statistically significant.4.The factor analysis of health administration's interests claims extracted four factors,their mean values were 4.003?4.659?4.126?4.505,P <0.01 for paired samples,and the mean difference was statistically significant.5.The factor analysis of medical insurance department's interests claims extracted four factors,their mean values were 4.374?4.269?3.881?4.144,P <0.01 for paired samples,and the mean difference was statistically significant.Conclusion 1.At present,the interests of the five types of stakeholders in the reform of public hospitals are more concerned with the interests of patients and their familie s,hospital employees,health administrative agencies,urban workers(residents)medical insurance institutions and new rural cooperative medical institutions.2.Patients with the interests of the hospital appeal for the "basic medical quality","medical and patient communication","reasonable medical expenses";with age,the "reasonable medical expenses" more and more attention;The degree of concern about "communication between doctors and patients" and "reasonable medical expenses" is relatively different.Farmers / migrant workers are most concerned about these two factors,and civil servants are least concerned.3.Hospital staff on the interests of the hospital appeal in turn "pay treatment","work atmosphere","career development space."4.the health administration of the interests of the hospital appeal for the "medica l institutions access management and informat ion disclosure","improve the manageme nt system","reform compensation mechanism","medical service system division of labor."5.medical insurance management departments of the interests of the hospital followed by "with the medical insurance work","improve the hospital interna l management","strengthen drug management","third party participation."...
