Clinical Study Of All-on-four Edentulous Fixed Denture Restoration

Objective: To study the method of implantation and repair of all-on-four edentulous mandibular denture and compared with overdentures which implant almost the same number of implanted implants in 6 aspects: the success rate of implants and prostheses;implant periodontal health(including peri-implant plaque index and bleeding on probing);masticatory efficiency;patient satisfaction survey;bone level changes around the implant.Methods: Collect patients with single jaw or double jaw edentulous,select those the production of ordinary complete denture(such as retention,stability,etc.)can not meet their needs in clinic,make all-on-four implant denture repair or implantation of overdenture repair for them,and observe the prognosis.The research steps are as follows:The two groups were followed by the following methods: Make radioactive guide plate,take CBCT,Computer design and production CAD / CAM guide.Under the guidance of the digital guide,A and B were implanted separately.Group A was implanted with 4 implants in the maxillary jaw according to the all-on-four procedure.The implanted temporary denture was made in the 24 h.The patient is permanently repaired after 3-6 months.Group B implanted four implants into the jaws of the patient under the positioning of the guide plate.After 3 to 6 months of osseointegration,the implant-supported overdenture was made.The clinical observation of the above two groups of patients A and B: Patient satisfaction was investigated in two groups of patients after 1 month of permanent repair.After 12 months of permanent repair,the two groups of patients were detection the success rate of restorations,implant periodontal health(including the implant around the plaque index and probe bleeding index),masticatory efficiency and bone height changes around the implant detection.Result:1.All-on-four edentulous implantation technique was to implant two axial implants in the anterior area and two oblique implants in the distal,and four implant-supported single-screw screws Bit of a fixed bridge.It only used four implants to achieve fixed restoration of edentulous jaw,reduced the patient's trauma and costs.All-on-four edentulous implant repair technology make patients wear the implant-supported temporary denture after the implant surgery immediately,solve the toothless situation during implant healing period.Patients can eat soft food and their life quality improved.2.1 month after permanent repair,we make denture satisfaction questionnaire for the patients,and the questionnaire showed that the score of all-on-four fixation techniques were higher than the implanted overdentures in chewing efficiency ? comfort ?conveniences and prononciation(P <0.05),the difference was statistically significant(P <0.05).3.12 months after permanent repair,the success rate of implant and restorations,implant periodontal health(including peri-implant plaque index and probe bleeding index),masticatory efficiency and bone level were detected.There was no significant difference between the two groups in success rate of implant and restorations ?implant periodontal health and bone level(P> 0.05).The results showed that there was no significant difference between the two groups(P> 0.05)The masticatory efficiency in group A is higher than in group B,the difference was statistically significant(P <0.05).Conclusion:1.All-on-four edentulous implant repair technique can be achieved by implanting only four implants.2.Compared to implanted overdenture,all-on-four edentulous implant repair technique achieves higher chewing efficiency and patients satisfaction.3.All-on-four edentulous implant restoration technique has no significant difference in the success rate of implants and restorations,the bone level around the implant.and the periodontal health(including peri-implant plaque index and probe bleeding index)compared with implantation of overdentures.
