Objective:The purpose of this study was to investigate the obstructive sleep apnea hypopnea syndrome(OSAHS)rate of glottic cancer patients,and the influences on OSAHS of different laryngeal reservation operations.Methods:124 patients were recruited in this study for an OSAHS screening with polysomnography(PSG)in portable monitor(PM),including 66 cases of glottic carcinoma patients without any treatment and 58 cases of glottic carcinoma patients treated with laryngeal reservation surgery and 6 months at least after tracheal decanulation.The criteria was set according to the “Guideline for the diagnosis and surgical treatment of obstructive sleep apea hypopnea syndrome” made by Editorial Board of Chinese Journal of Otorhinolaryngology Head and Neck Surgery Subspecialty Group of Pharyngolaryngology,Society of Otorhinolaryngology Head and Neck Surgery,Chinese Medical Association.We divided those patients with laryngeal reservation surgery into two subgroups based on different surgical range.The severity of OSHAS was evaluated with the value of apnea-hypopnea index(AHI),and hypoxemia was judged with the lowest blood oxygen saturation(LSa O2).Results were then analyzed statistically.Results: 38 in 66 cases(57.57%)of glottic carcinoma without treatments,and 32 in 58 cases(55.17%)of glottic carcinoma with laryngeal reservation surgery were diagnosed as OSAHS.There was no significant difference between them(X2=0.073,P>0.05).However,there was 3.03% and 13.79% patients were diagnosed severe OSAHS respectively,there was statistical significance between the two groups for diagnostic rate of severe OSHAS patients(X2=4.823,P<0.05).Patients performed with wider operational range showed higher diagnostic rates whether in OSHAS(X2=5.393,P<0.05)or severe OSHAS(X2=5.130,P<0.05)compared to patients with smaller range of resection.Conclusion:According the results,glottic carcinoma patients without treatment showed higher OSAHS incidence;Patients treated with laryngeal reservation also showed high incidence of OSAHS,which was aggravated after operations.The range of damage in laryngeal structure was positively related with the severity of OSHAS.In further study,sleep-disordered breathing problem of golottic carcinoma patients without treatments or with laryngeal reservation opeartions would be concerned.
