Background Statins have plenty of other impacts named “ pleiotropic effects ” besides inhibiting cholesterol synthesis and reducing serum lipid.Leptin,one of proteohormones secreted by adipocytes,can exacerbate athesclerosis progression.Some Studies have reported that statins can inhibit leptin expression and decrease serum leptin level,whereas some othe researches come to an opposite conclusion.This inconsistency in research results probably are associated with lep gene promoter 2548G/A polymorphism which regulates leptin gene expression.One study shows lep gene promoter 2548G/A can influence the lipid-lowering effects of statins.In clinical practice,the individual difference in lipid-lowering amplitude after statins therapy maybe related to 2548G/A polymorphism.This study is to explore the relationship between lep gene promoter 2548G/A polymorphism and the effects on serum leptin and lipid of atorvastatin.Method In this study we recruited 48 acute cerebral infarction patients' fasting blood(EDT A anti-freezing)visiting our hospital(total 48,male 29,female 19).All were analyzed the genotypes through extracting DNA from white cells by the method of polymerase chain reaction combined with restrictive fragment length polymorphism.We measured the levels of serum leptin(by enzyme linked immunosorbent assay)and lipid(by automatic biochemical detector)before and after atorvastatin treatment(Lipitor,Pfizer,20mg/day,for 7days).We analyzed the alteration of serum leptin and lipid after atorvastatin therapy,and explore the relationship between these alteration and lep gene promoter 2548G/A polymorphism.Results The serum basic leptin level in group AG/GG was significangtly higher than that in the group AA(X±S,?/ng·ml-1,AA : 9.29±7.66,AG/GG : 13.68±7.31,p<0.05).In the total population(n=48),the alteraion in leptin befor and after atorvastatin did not have any significance(?X±S,?/ng·ml-1,before : 11.30±7.74,after : 11.47±7.91,p>0.05),but when the population was divided to two groups according to genotypes,the datas show the serum leptin in the AG/GG group was significantly reduced(X±S,?/ng·ml-1,before : 13.68± 7.31,after : 13.30±7.95),whereas in the AA group the leptin level was not decreased significantly(?X±S,?/ng·ml-1,before : 9.29±7.66,after : 9.79±7.65,p>0.1).In the total population(n=48),Atorvastatin significantly reduced total cholesterol and low density lipoprotein cholesterol(p<0.001)in the serum,but no significant distinction in rangeability was observed between group AA and group AG/GG(p>0.05).A negative correlation between leptin and HDL-C did exist in the non overweight(r=-0.44,p<0.045).Conclusions Atorvastatin can gene-dependently reduce leptin expression,but its lipid-lowring effects do not have an association with 2548G/A polymorphism.
