Objective To research the emotional and behavior problems of children with epilepsy with monotherapy,and to analyze the different effects of antiepileptic drugs on emotion and behavior,which would give a better guidance when choosing antiepileptic drugs.Methods Children with idiopathic epilepsy and treated with topiramate(TPM),levetiracetam(LEV),valproate(VPA)for monotherapy from September 2014 to June 2016 in our hospital were collected in as the case group.And the newly diagnosed children with epilepsy as the control group.Then use the Achenbach's child behavior checklist to evaluate the emotional and behavior problems.The results were compared to analyze every sets of children general conditions,incidence of abnormal behavior problems,every factor scores and the total CBCL scores.A p value of less than 0.05 was viewed as significantly.Results(1)The total incidence of abnormal behavior problems in children with epilepsy is 7.7%.Incidence of abnormal behavior problems in every group: LEV(11.5%)>TPM(9.1%)>N ewly diagnosed group(7.1%)>VPA(3.4%).There was no statistically significant difference among every group(?~2=1.344,P>0.05).No special difference of abnormal behavior rate between male(10.7%)and female(2.9%)in children with epilepsy(?~2=1.873,P>0.05).(2)The scores of each group were compared by four different sex age groups,the scores from high to low is as follows:(1)6-11-year-old male children with epilepsy in division and compulsive,the LEV group is higher than VPA group,TPM group,newly diagnosed group and health control group.In physical complaints and discipline,the TPM group is higher than VPA group,LEV group,newly diagnosed group and health control group.In depression,poor communication,social withdrawal,hyperactivity,aggression,and CBCL scores,the TPM group is higher than LEV group,VPA group,newly diagnosed group and health control group.6-11-year-old male children with epilepsy in hyperactivity,depression,aggression,CBCL scores were statistically significant(P<0.05).The scores of hyperactivity in TPM group is higher than VPA group,newly diagnosed group and health control group,The scores of depression in TPM group is significantly higher than newly diagnosed group and health control group,the aggressive scores and the total CBCL scores of TPM,VPA and LEV are higher than health control group.(2)6-11-year-old female children with epilepsy in depression and physical complaints,the LEV group is higher than TPM group,VPA group,newly diagnosed group and health control group.In social withdrawal,hyperactivity,aggression,and CBCL scores,the TPM group is higher than LEV group,VPA group,newly diagnosed group and health control group.In division and compulsive,the TPM group is higher than LEV group,newly diagnosed group,VPA group and health control group.The 6-11-year-old female children with epilepsy in hyperactivity,depression,aggression,CBCL scores were statistically significant(P<0.05).The scores of depression in TPM and LEV group is significantly higher than VPA group,newly diagnosed group and health control group,the aggressive scores and the total CBCL scores of TPM and LEV is higher than VPA,newly diagnosed group and health control group.(3)12-16-year-old male children with epilepsy in physical complaints,the VPA group is higher than newly diagnosed group,TPM group,LEV group and health control group.In division,poor communication,compulsive,discipline,aggression,and CBCL scores,the TPM group is higher than VPA group,LEV group,newly diagnosed group and health control group.The scores of poor communication in 12-16-year-old male children with epilepsy is higher than health control group.(4)The scores of 12-16-year-old female children with epilepsy has no statistically significant difference(P>0.05).Conclusions(1)The 6-11-year-old epilepsy patients treated with topiramate were more prone to emotional behavior compared with taking levetiracetam and valproic acid,mainly manifested in hyperactivity,social withdrawal,discipline,aggression.The incidence of emotional behavior problems in children with epilepsy aged 12-16 years was relatively lower,mainly manifested in schizophrenia,poor communication,compulsive,discipline and aggression.(2)The emotional behavior problems of children aged 6-11 years treated with levetiracetam monotherapy were lower than those of the topiramate group,but the division,compulsive of male children and depression,physical complaints of female children were higher than topiramate and valproic acid group.12-16 years of age the incidence of children with epilepsy is relatively reduced,mainly for hostility,anxiety,depression,aggression and so on.(3)The prevalence of emotional behavior problems in children aged 6-11 years treated with valproate was less likely to occur,relatively common with schizophrenia,compulsive,physical complaints,disciplin,but the incidence of 12-16 age groups was relatively increased,mainly manifested as immature,physical complaints,hyperactivity and division.(4)There was no significant difference in the incidence of emotional behavior among children with different sexes.
