Objective:The purpose of this study is to investigate the clinical features of patients with both subclinical hypothyroidism and type 2 diabetes,and to analyze its relation with diabetic vascular complications.This paper is expected to be a useful reference work which may provide new ideas for early prevention and treatment of T2 DM vascular complications.Methods:1)Patients totaled 200 with type 2 diabetes mellitus treated in department of endocrinology of Affiliated Hospital of Qing Hai University during November 2015 to November 2016 were collected as samples of the study.Thereinto,according to the SCH diagnostic criteria,97 patients with subclinical hypothyroidism were treated as T2 DM with SCH group and the rest 103 patients with normal thyroid as simple T2 DM group.2)The general information and the routine biochemical test indicators of these patients were collected.The intima-media thickness of blood vessels was measured in bilateral carotid color Doppler ultrasonography inspection,at the same time plaque formation could be recorded.ABI,the ratio of ankle artery pressure to brachial artery pressure,was calculated by the color Doppler ultrasound test results.Besides,eyes of all patients were examined by ophthalmic consultation.3)Comparing and analyzing the general data and clinical data of the two groups,this paper summarized the clinical characteristics of patients with T2 DM combined with SCH,and analyzed the differences of these indexes and their effects on DM vascular complications.SPSS21.0 system software was applied in statistical processing and analysis of data.Results: 1.The level of TC,TG,24 h urinary protein,ACR and TSH in T2 DM combined with SCH group were higher than those in T2 DM group,and the level of Hb A1 c,FT3 and FT4 were lower than those in group T2DM(P<0.05).2.Compared with T2 DM group,the incidence rate of DN,CAS and PAD in T2 DM combined with SCH group was higher,and the difference was statistically significant(P<0.05).But there was no significant difference in the incidence rate of diabetic retinopathy between the two groups(P>0.05).3.Single factor and multiple factor logistic regression analysis showed that SCH was a risk factor for DN,CAS and peripheral arterial disease of lower extremity.Conclusion:The incidence of subclinical hypothyroidism in patients with type 2 diabetes is high,which to various degrees impacts glucose metabolism and lipid metabolism of patients and is a risk factor of diabetic nephropathy,carotid atherosclerosis and peripheral arterial disease of lower extremity exist.
