Diabetes mellitus has become one of the greatest threats to human health in the word.Erectile dysfunction is one of the complications of diabetes, which is accounting for 75% of diabetic male. With the increase in the number of diabetes, incidence of diabetic complications will also increase, including erectile dysfunction(Erectile dysfunction ED)which is most closely related with diabetes and seriously affecst the quality of life of patients.Owing to its complex neurovascular aetiology, diabetic ED is hard to treat, and it does not respond well to oral treatment with phosphodiesterase type 5 inhibitors, which are currently the first-line therapy for treating ED.An improved understanding of the molecular mechanisms involved in the pathogenesis of diabetic ED is,therefore, essential to aid in the development of effective targeted therapies. The pathogenesis of diabetic ED is multifactorial, and involves changes in endothelial dysfunction,NO/CGMP pathway,Rho A/Rho pathway, hyperinsulinemia, oxidative stress, and advanced glycation end products. Therefore,in the process of clinical diagnosis and treatment of diabetes, the acknowledge of the mechanism of diabetic erectile dysfunction and attention to the treatment of ED,which can improve outcomes and the quality of life, are of great significance.
