Objective: Extramammary Paget's disease(EMPD)was first reported in1989 by Crocker,which is a rare skin malignant tumor,and the histological origin still remains controversial.The disease mostly occurs in the elderly,and which is common in the anus,external genital area.It's characterized by eczematous lesions in clinical and Paget's cells in histological.Tumor formation is often accompanied by inactivation of anti oncogene and up regulation of oncogene expression,and the relationship between inflammation and cancer has been paid more and more attention.Inflammation can change the micro environment,increase the probability of cell mutation,and promote tumor.NF-?B and Nrf2 signaling pathway not only plays an important role in oxidative stress,but also in some malignant tumors,the expression of NF-?B and Nrf2 can also be found.To detect the expression of tumor suppressor gene PDCD4 and oncogene Survivin,NF-?B and Nrf2 in EMPD lesions and normal tissues by immunohistochemistry,and to discuss the role of PDCD4,Survivin,NF-?B and Nrf2 in EMPD occurrence and development.Methods: A total of 30 cases of EMPD tissues confirmed by pathology were collected from January 2000 to May 2014 in Cangzhou City Central Hospital.20 cases of normal tissues of the axillary tissue were selected as the control group.The difference of the positive expression rates of PDCD4,Survivin,NF-?B and Nrf2 in diseased tissues and normal tissues was compared by immunohistochemical technique and comprehensive staining intensity and staining rate.SPSS17.0 statistical software was used to process the data,and the threshold value with statistically significant difference was set to P <0.05 by ?2 test.PDCD4,Survivin,NF-?B and Nrf2 were analyzed by Pearson correlation analysis.Result:1 The positive rate of PDCD4 in EMPD tissues was 23.3%,and the positive rate was 100% in normal tissues.There was significant difference between the two groups(P <0.05).The positive rate of Survivin in EMPD tissues was 80%,and it was not expressed in normal tissues.There was significant difference between the two groups(P <0.05).The positive rate of NF-?B in EMPD tissues was 70%,and it was not expressed in normal tissues.There was significant difference between the two groups(P <0.05).The positive rate of Nrf2 in EMPD tissues was 73.3%,and the positive rate in normal tissues was 10%.There was significant difference between the two groups(P <0.05)2 The positive rates of NF-?B in Survivin positive and negative patients were 83.3% and 33.3%,the difference between the two groups was statistically significant(r=0.452 P=0.013).The positive rates of Nrf2 in Survivin positive and negative patients were 79.2% and 33.3%,the difference between the two groups was statistically significant(r=0.4 P=0.028).3 The positive rates of PDCD4 in patients with positive and negative Survivin were 12.1% and 66.7%,the difference between the two groups was statistically significant(r=-0.512,P=0.005).The positive rates of PDCD4 in NF-?B positive and negative patients were 4.8% and 66.7%,the difference between the two groups was statistically significant(r=-0.671,P=0.001).The positive rates of PDCD4 in Nrf2 positive and negative patients were 9.1% and62.5%,the difference between the two groups was statistically significant(r=-0.588,P=0.002).Conclusion:1 The expression of PDCD4 protein in Extramammary paget's disease was significantly lower than that in normal tissues.2 Survivin,NF-?B and Nrf2 proteins were significantly higher in the Extramammary Paget's disease than in normal tissues.3 Survivin protein was positively correlated with NF-?B and Nrf2 protein expression in Extramammary Paget's disease.4 PDCD4 protein was negatively correlated with Survivin,NF-?B andNrf2 protein expression in Extramammary Paget's disease.5 Tumor suppressor gene PDCD4,oncogene Survivin and NF-?B,Nrf2 involved in the occurrence and development of Extramammary Paget's disease,and the inflammatory response may play an important role in the formation of Paget's disease.6 Survivin,NF-?B and Nrf2 protein can be used as immunohistochemical markers to assist in the diagnosis of Extramammary Paget's disease,and can help to clarify the postoperative margin.
