Acute cerebral ischemic stroke is the most common type of cerebrovascular diseases,which can lead to neurological deficits.AIS can result in serious damage to the physical and mental health of patients and lower the quality of life in old people.Relevant data show that the hospitalized patients with AIS each year in our country up to 45 million,which accounts for about 78.9% of patients in neurology ward,and the incidence of adverse sequelae,such as limb paralysis,speech disorder and so on,can reach to about 90.3% and has a trend of increase year by year.As the further study of pathogenesis of AIS,it is gradually clear that the characteristic spectrum of inflammation after ischemic stroke.And independently find that inflammatory injury is a key pathological mechanism of secondary injury of brain tissue after ischemic cerebrovascular disease.Atherosclerosis is one of the risk factors of ischemic cerebral stroke,and is widely identified as chronic inflammation of blood vessel wall.The formation or shedding of atherosclerotic plaques will lead to reduced distal vascular perfusion.Also the instability of plaques and the internal hemorrhage or necrosis can further lead to thrombosis.With atherosclerotic plaques,patients may suffer a high incidence and recurrence rate of AIS.Therefore,there is a close connection between inflammation,atherosclerosis and AIS.In recent years,it has been found that berberine,one of the Chinese traditional medicine,has the effects of anti tumor,anti arrhythmia,regulating glucose and lipid metabolism and so on.The application research of berberine in the field of cerebrovascular diseases has become a research focus.120 cases of patients with AIS were selected in the study and were divided into berberine group and general group according to completely randomized design.The study records the change of the levels of inflammatory factors before and after 14 days' treatment,at the same time collect the information of carotid duplex ultrasound of patients before and after 90 days' treatment.The study aims to discuss the effect of berberine on AIS through a comparative research on the clinical efficacy.Objective:This study aims to analyze the effects of berberine on serum inflammatory factors and carotid atherosclerotic plaques in patients with acute cerebral ischemic stroke(AIS).Methods:In the study,120 patients with AIS met the criterion were randomly separated into berberine group(n=60)and general group(n=60).The 60 cases in the general group were provided with general therapy according to the latest guidelines of diagnosis and treatment of AIS.And berberine group additionally received berberine 300mg(tid)on the basis of general group.The levels of serum inflammatory factors(macrophage migration inhibitory factor,MIF;interleukin-6,IL-6;c-reactive protein,CRP),the nerve function defect grades(National Institute of Health Stroke Scales,NIHSS scores;modified Rankin Scale,mRS scores)and the indexes of carotid atherosclerosis plaques(the total plaque area,TPA;the crouse integral;intima-media thickness,IMT;the number of unstable carotid atherosclerotic plaques)were measured and compared.Results:The results indicated that the levels of serum inflammatory factors,NIHSS cores and the indexes of carotid atherosclerosis plaques were no significant differences between the two groups on day1(P>0.05)and there was positive correlation of serum inflammatory factors and NIHSS scores(P<0.05).The levels of serum inflammatory factors and NIHSS scores of the two groups on day14 were significantly lower than those on day1(P<0.05),and the differences are statistically significant.The levels of serum inflammatory factors and NIHSS scores of the berberine group on day14 were significantly lower than those of general group(P<0.05),and the differences are statistically significant.The TPA,the crouse integral and the number of unstable carotid atherosclerotic plaques of the two groups on day90 were significantly lower than those on day1(P<0.05),and the differences are statistically significant.The IMT had the trend of lower,but there was no significant differences(P>0.05).The TPA,the crouse integral and the number of unstable carotid atherosclerotic plaques of berberine group on day90 were significantly lower than those of general group(P<0.05),and the differences are statistically significant.The mRS scores of berberine group on day90 were significantly lower(P<0.05)and the rate of short-term favorable prognosis was significantly higher(P<0.05).There was no significant differences in the incidence of adverse reactions between the two groups.Conclusions:This study showed that berberine therapy on the basis of general treatment can significantly lower the levels of serum MIF,IL-6 and CRP,reduce the degree of carotid atherosclerosis to some extent,improve the related indexes of carotid atherosclerotic plaques,and improve neurological impairment and the prognosis of patients with AIS.
