Objective: To explore the role of DCs in the pathogenesis of Chronic obstructive pulmonary disease(COPD)by Th17/Treg pathway through observing the changes of mature dendritic cells(m DCs),immature dendritic cells(im DCs),Helper T cells 17(Th17)and Regulatory T cells(Treg)in lung tissue and peripheral blood of COPD patients.Methods: According to the patients’ medical records in thoracic surgery of our hospital who required the lobectomy during Sep.2015 to Mar.2016,on the basis of GOLD’s standard in 2015 fit them into non-smoking no COPD group,smoking but no COPD group and COPD group.Their lung tissue was collected(more than 5 cm from the lesion margin).The data of hospitalized patients were collected from January to December of 2016 in our hospital and patients were divided into Acute exacerbation of COPD group(AECOPD),stable COPD group(SCOPD)according to 2016 GOLD standard.Collectted the Middle-aged people of Physical examination department and divided into non-smoking no COPD group(CS-COPD-),smoking but no COPD group(CS+COPD-).Then collected their peripheral venous blood.The pathomorphology of patients’ lung and bronchiole were investigated by HE staining,pathological semi-quantitative score to assess the pulmonary pathology.The expression levels of CD80,CCR6,IL-17 A and Fox P3 were detected by immunohistochemistry(IHC).The changes of m DCs,im DCs,Th17 cells and Treg cells in lung tissue and peripheral blood were detected by flow cytometry(FCM)and the correlation with lung function was analyzed.Results: 1.The obvious chronic bronchitis and emphysema pathological changes can be seen in the lung tissue of patients with COPD,and small airway pathology semiquantitative score is higher than those without COPD with a significant difference(P<0.05).2.The IHC results showed that the expression of CD80 and Fox P3 in COPD group was lower than that in CS-COPD-group and CS+COPD-group(P <0.05).The expression of CCR6 and IL-17 A was higher than that in CS-COPD-group and CS+COPDgroup with a significant difference(P <0.05).3.The FCM results showed that the percentage of m DCs and Treg in lung tissue of COPD group was significantly lower than that in CS-COPD-group and CS+COPD-group(P <0.05).The percentage of im DCs and Th17 in COPD group was significantly higher than that in CS-COPD-group and CS+COPD-group and the difference was statistically significant(P <0.05).4.The FCM results showed that the percentage of m DCs and Treg in peripheral blood of AECOPD group and SCOPD group was significantly lower than that in CS-COPD-group and CS+COPD-group(P <0.05).The percentage of im DCs and Th17 in AECOPD group and SCOPD group was significantly higher than that in CS-COPD-group and CS+COPDgroup and the difference was statistically significant(P <0.05).5.The imbalance of Th17/Treg exists in peripheral blood and lung tissue of patients with COPD,and tend to be the Th17 cell-mediated proinflammatory response.6.FEV1%,FVC%,FEV1/FVC had no correlation with im DCs,m DCs,Th17 cells and Treg cells in lung tissue and peripheral blood of COPD patients.Conclusion:To detect the Changes of DCs and Th17 / Treg cells in lung tissue and peripheral blood of patients with COPD by IHC and FCM,the results show that COPD patients with m DCs decreased,im DCs increased,indicating that DCs mature disorders.Th17 cells in COPD are increased,and Treg cells are decreased,so there is Th17/ Treg imbalance,may be involved in the pathogenesis of COPD.
