Objectives By measuring serum chitinase 3-like protein1,blood glucose,blood lipid and other indicator in patients with type 2 diabetes mellitus and diabetic retinopathy,analysis the risk factors of diabetic retinopathy,and explore the relationship between chitinase 3-like protein1,blood glucose,blood lipid and type 2 diabetic retinopathy.Provide the basis for early diagnosis and treatment of diabetic retinopathy.Methods Select 122 cases of type 2 diabetes patients from December 2015 to December 2016 in the department of endocrinology of affiliated hospital of North China University of Science and Technology.According to fundus camera and fluorescence fundus angiography,the 122 samples are divided into two groups:77 cases who have diabetic retinopathy(DR group)and chose 45 cases who have diabetes without retinopathy(DM group)as control group.DR group include diabetes with non proliferative retinopathy(NPDR group)45 cases and 32 cases who have proliferative retinopathy(PDR group).Record gender,age,course of the disease,height,body weight,body mass index(BMI)and measure systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting venous blood glucose(FPG),glycosylated hemoglobin(Hb Alc),fasting insulin(FINS),fasting C peptide(FC),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein(LDL-C),urine trace albumin(UAlb),hypersensitive creactive protein(hs-CRP)and chitinase 3-like protein1(YKL-40)level of samples,and compute the homa insulin-resistance(HOMA-IR).Make questionnaire,collect data,analyse all the data with SPSS23.0 statistical software package.Measurement data is shown in the form of((?)ąs).Count data is measured by chi-square test,measurement data comparison using t test between groups;The relationship between the probability of DR and all indicators of interest is examined by the Logistic regression which indicates some statistically significant relationships with P<0.05.Pearson correlation correlation and multiple stepwise regression method are applied to observe the relationship between YKL-40 and other indicators.Results 1 The difference in gender composition among two groups was not statistically significant according to the c2 test(P>0.05),which guarantees the validity of comparison among groups.2 The difference of age,BMI,DBP,FINS,FC between two groups has no statistical significance by t test(P> 0.05).3 The course of type 2 diabetes,SBP,FPG,Hb A1 c,HOMA-IR,TC,TG,LDL-C,hs-CRP,UA1 b in DR group higher than DM group,HDL-C in DR group lower than DM group,the difference was statistically significance(P<0.05).4 The level of serum YKL-40 in DR group was higher than that in DM group,and increases with the development of diabetic retinopathy,the difference was statistically significant(P<0.05).When the level of YKL-40?118ng/ml,the risk of T2 DM with DR increased 16.64 times.When the level of YKL-40?157ng/ml,the risk of NPDR develop into PDR increased 21.63 times.5 Using the occurrence of diabetic retinopathy as dependent variable,the t test statistically significant variables as independent variables,the logistic regression results show DR is positively correlated with course of disease,Hb A1 c,YKL-40 and UA1b(P<0.05).6 Using YKL-40 as dependent variable,the correlation method shows that YKL-40 is positively correlated with Hb A1 c,course of disease,UA1 b,hs-CRP,FPG,TG,LDL-C,TC,HOMA-IR,SBP(P<0.05),negatively correlated with HDL-C(P<0.05);The multiple stepwise regression analysis with YKL-40 as dependent variable and course of disease,SBP,FPG,Hb A1 C,HOMA-IR,TC,TG, HDL-C,LDL-C,hs-CRP,UA1 b as independent variables shows YKL-40 is positively correlated with course of disease,Hb A1 c,hs-CRP,UA1b(P<0.05),negatively correlated with HDL-C(P<0.05).Conclusions 1 The YKL-40 level of patients with DR is significantly higher than that without DR,and YKL-40 level increases with the development of DR.2 The level of serum YKL-40 was associated with the occurrence and development of DR,the level of serum YKL-40 increased,the risk of occurrence and development of DR increased.
