The Relationship Between Serum Ferritin And Glucose,Lipid And Purine Metabolism In The Patients With Gout/hyperuricemia

Objective This study aimed to investigate the effects of serum ferritin levels on uric acid,glucose and lipid metabolism in healthy subjects,patients with hyperuricemia and gout,and to explore the possible role of serum ferritin in the development of gout,and then to provide reference for the diagnosis and treatment of gout.The dietary intake and behavioral changes of male patients with gout were also researched.Methods In this study,more than 2 920 male patients with gout,hyperuricemia and healthy people were selected as the research objects from some medical and physical examination institutions in Qingdao by means of observational epidemiology;of which 600 cases of gout patients were selected as case group,meanwhile 1 200 male healthy people of the same period were randomly selected as the control group and 1 120 male patients with hyperuricemia as hyperuricemia group.We collected the detection information of physical examination and related indicators of the three groups,such as height,weight,serum ferritin(SF),high sensitive C reactive protein(hs-CRP),serum uric acid(UA),triglyceride(TG),total cholesterol(TC)and fasting plasma glucose(FPG).Meanwhile,we conducted a dietary and behavioral survey of 430 male patients with gout in a hospital.Results There were differences among BMI,TG,TC,FPG,UA,SF and hs-CRP of the three groups(P<0.001).Adjusted for age,BMI,SUA,FPG,TG and TC,the partial correlation analysis showed that the SF was positively correlated with SUA,FPG,TG and TC in the three groups(all P<0.01),and the correlation between SF and the indexes in the gout group was higher than that in the hyperuricemia group(all P<0.05).In the gout group,hs-CRP and uric acid levels were positively correlated(r=0.274,P<0.05),and SF and hs-CRP levels were positively correlated(r=0.191,P<0.001).Multivariate Logistics regression analysis showed that the high level of SF(>160.95 ?g/L)had significantly increased risk of hyperuricemia,with OR of 2.420(95%CI,1.068~5.483);however hs-CRP was not a risk factor for hyperuricemia.SF >107.65 ?g/L and hs-CRP >1.00 mg/d L had significantly increased risk of gout,with ORs of 1.627 and 5.879(95%CI,1.052~2.515 and 3.521~9.814).In the hyperuricemia group,SF>160.95 ?g/L is a risk factor for FPG(OR=1.956,95%CI=1.270~3.013),TG(OR=1.678,95%CI=1.190~2.364),and TC abnormalities(OR=1.622,95%CI=1.118~2.352).However,in the gout group,SF>107.65 ?g/L is a risk factor for FPG(OR=1.482,95%CI=1.025~2.142)and TG abnormalities(OR=1.815,95%CI=1.035~3.184),and SF>160.95 ?g/L is a risk factor for TC abnormalities(OR=2.422,95%CI=1.340~34.379).46.98% of patients with gout didn't exercise,and if exercise,the main form of exercise was walking.The average daily intake of livestock and poultry in gout patients was 97.08 g,which was significantly higher than the recommended dietary intake(40-75 g/d).The intake of vegetables(248.80 g/d)and fruit(168.33 g/d)were not up to the national recommended dietary intake,and the intake of dark vegetables(95.04 g/d)was significantly lower than that of light colored vegetables(153.75 g/d).The intake of milk and its products,soybean and its products and fish were 80.24 g/d,12.76 g/d and 21.05 g/d,which all could not meet the recommended intake of dietary guidelines for Chinese Residents.The proportions of patients who drank white wine,red wine and beer were only5.35%,0.70%,and 1.40% every day,respectively.Conclusion The level of activity in patients was affected by gout,who also reduced the intake of high purine foods(animal offal,seafood)and alcohol,while the intake of vegetables,fruits and dairy products in gout patients was still inadequate.It was found that high level of SF was a risk factor of hyperuricemia/gout,and the level of SF was closely related with disorder of glucose and lipid metabolism in patients with gout and hyperuricemia,which suggested that actively control SF might be beneficial to the improvement of glucose and lipid metabolism.In conclusion,the detection of SF in susceptible populations could be used as a new marker to predict hyperuricemia and gout in the future.
