Purpose:Analysis of normal thyroid function in patients with type2diabetesmellitus (T2DM) of thyroid hormone (TH) level and blood lipid, blood glucose,the metabolism of purine, aims to further find the impact of adverse prognosticfactor in diabetes mellitus.Method:Three hundred and eighty three patients with type2diabetes selected whostayed in Department of Endocrinology of Frist Hospital of JiLIin Universityfrom in January in2011to December in2011, male177cases, female206cases.The above selected patients had normal thyroid function, eliminateknown hyperthyroidism including subclinical thyroid function, thyroiddysfunction including subclinical hypothyroidism,and severe infections,malnutrition leads to low T3syndrome,thyoid stimulating hormone were in therange of0.27-4.2mU/L. Collect patients with clinical data including sex, age,course of disease; measurement of blood pressure, height, weight, fastingplasma glucose (FPG), uric acid (UA), serum total cholesterol (TC),triglycerides (TG), high density lipoprotein cholesterol (HDL-C), low densitylipoprotein cholesterol (LDL-C), glycosylated hemoglobin (HbA1C), three freetriiodothyronine (FT3), free thyroxine (FT4), thyroid stimulating hormone(TSH). Compare the difference of thyroid hormone level between differentgender, disease duration and blood glucose groups in type2diabetes, and observe the correlation between thyroid hormone and blood lipid and glucose,the metabolism of purine.Result:1. Comparison of clinical data of different gender group, female patientswas significantly higher than that of male patients with TSH, FT3、FT4、 UAwere significantly lower than those of male patients.2. According to the level of HbA1c were divided into three groups, therewere significant difference between different groups of TSH (P<0.05);according to the different levels of TSH were divided into three groups, therewas significant difference between different groups of TC, TG(P<0.05);compared with the1.6mIU/L-2.9mIU/L group,2.9mIU/L-4.2mIU/L group,UA and LDL-C were significantly increased (P<0.05).3, TSH was negatively correlated with HbA1c (r=-0.122, TG, P=0.034),and LDL-C, UA, TC, positive correlation (r=0.177,0.131,0.124,0.142,P=0.001,0.012,0.023,0.007), FT3and HbA1c, FPG, age was negativelycorrelation (r=-0.12,-0.120,-0.228, P=0.011,0.025,0.000). Independentfactors affecting the change of FT3age and FPG (OR=-0.181,0.266.P=0.001,0.003).Conclusion:1.In patients with T2DM, hyroid hormone has effect on blood glucose,blood lipid and evaluation of purine metabolism.2, Elevated blood glucose levels can lead to decreased FT3and TSHlevels.3, FT3is a predictor of poor prognosis of patients with diabetes mellitus.4, The changes of thyroid function in regular monitoring with implicationson patients with T2DM.
