Objective: To explore the effect of cocktail therapy on early postoperative pain after total knee arthroplasty(TKA).Through comparing the VAS score,patients' knee joint maximum buckling degree,the time required to reach the straight leg-raising function,remedial opioid usage and incidence of adverse reactions at different period,we discussed the early analgesia and knee function recovery effect of “cocktail” therapy after total knee arthroplasty,provided theoretical basis for safe and effective analgesia which is beneficial to patients' early knee function recovery after total knee arthroplasty.Methods: 61 patients with TKA treated in our hospital from January 2016 to December 2016 were divided into the study group(31 cases)and the control group(30 cases).The control group use conventional analgesia and the study group use "cocktail" therapy besides conventional analgesia.Intravenous self-control analgesia pump should be used in two groups of postoperative analgesia intervention,for two days;Diclofenac sodium enteric-coated metformin hydrochloride was taken six hours after surgery,for three days.Compared two groups of patients with postoperative resting state 6 h,12 h,24 h,48 h,72 h,activity condition after the operation,24 h,48 h,72 h of knee pain,24 h,48 h,72 h of knee joint maximum buckling degree and the time required to reach the straight leg-raising function after the surgery.Compared two groups of patients of remedial opioid usage within 24 h after the operation.Analyze two groups of patients with postoperative adverse reactions occur.All data processing,uses SPSS17.0 software,count data uses“x2 ”test;Measurement data uses “t ”test.Results: The VAS score of the study group in quiescent state at 6h,12 h,24h,48 h after operation are lower than that of the control group,P<0.05,with statistically significant;in active state,VAS score of the study group at 24 h and 48 h is lower than that of the control group,P<0.05,with statistically significant.The study group after the operation condition of knee joint maximum buckling degree is higher than the control group P<0.05,with statistically significant.The time required to reach the straight leg-raising function after the operation,the study group is shorter than the control group,P<0.05,with statistically significant.Remedial opioid usage within 24 h,the study group is lower than the control group,P<0.05,with statistically significant.There was no statistical difference in adverse reaction between the two groups(P>0.05).Patients of the two groups after all did not appear the symptom of organ damage such as heart,lung,brain and kidneys.All patients have indwelling catheter postoperatively for 12 h,and didn't appear urinary retention.Conclusion: "Cocktail" therapy in the treatment of early postoperative TKA pain is safe and effective.Local injection of "cocktail" after total knee arthroplasty could effectively reduce early postoperative pain and speed up the recovery of knee joint function."Cocktail" therapy has obvious application effect in early postoperative period after TKA.
