Objective: In recent years,China's elderly population in the incidence of chronic kidney disease(CKD)increased year by year in 2011,the United States kidney registration system statistics show that,2005-2008 11% of citizens have CKD,and suffering from CKD The number of patients is still increasing year by year. And CKD patients with secondary hypertension as high as 86.2%,so the pathogenesis of CKD and secondary hypertension pathogenesis of the two diseases will be the development and prognosis is particularly important,CKD patients with cardiovascular disease(cardiovascular disease Disease,CVD)risk is 10 to 30 times higher than the average population, although the medical treatment of kidney disease has been developed by leaps and bounds, the treatment method has also been greatly improved, but CVD is still CKD patients, especially Is the most common complication and first cause of death in patients with end stage renal disease(ESRD).Arterial stiffness is not only closely related to cardiovascular risk factors but also to cardiovascular disease.Who were followed up for 112 months in 1980 from 1996 to 1996,showed that arterial stiffness was significantly associated with all-cause mortality and cardiovascular mortality in hypertensive patients,After a classic cardiovascular risk factor, arterial stiffness still has a strong predictive value.Professor Asmar said that arterial stiffness is conducive to the identification of early vascular disease,it is worth to widely used in clinical practice.Early detection of vascular lesions and arterial dysfunction will be the ultimate reduction in cardiovascular and cerebrovascular disease mortality and morbidity of the important measures.Arterial stiffness is one of the indicators that is currently considered to be an effective measure of cardiovascular disease in patients with CKD,and arterial stiffness in patients with chronic kidney disease with hypertension is essential.Measure the arterial stiffness and cardiovascular disease of the gold index is the pulse wave velocity, which is characterized by noninvasive, easy to operate, can be measured and no adverse reactions to patients in recent years has been widely used in clinical. The purpose of this study is to investigate the arterial stiffness in patients with chronic kidney disease combined with hypertension,so as to further investigate the effect of the pulse wave velocity on the cardiovascular system. Methods: 1.The study selected 200 patients with CKD from December 2015 to December 2016 in the Department of Nephrology of our hospital as the patients group.2.We selected 26 normal personnel in the outpatient physical examination in the same period as the healthy control group, the normal liver and kidney function, and all the healthy control group were informed consent.3.The laboratory test results of all populations: creatinine,Scr,total cholesterol,TC,glycerin three,TG.4,using a standard mercury sphygmomanometer cuff measurement of arteriovenous fistula in the lateral upper arm sitting blood pressure, pulse wave velocity measurement instrument detection of PWV ACF-V1.Results: 1,CKD patients group SBP,PP,PWV significantly higher than hypertension patients with SBP,PP,PWV is also significantly higher than non hypertensive patients,P < 0.05,statistical significance.2,CKD hypertensive patients in the group of age,SBP,PP,e GFR,PWV differences,we can find that with the progress of CKD,kidney damage increased,PWV showed a significant upward trend,P < 0.05,statistically significant. The difference of age,SBP,PP,e GFR and PWV between the groups of 3 and CKD non hypertensive patients can be found that with the progress of CKD and the aggravation of kidney damage,PWV showed a significant upward trend,P < 0.05,statistically significant.4,different GFR groups age,SBP,DBP,PP,e GFR and PWV are different, with the aggravation of renal injury, the progress of CKD,from CKD1 to 5,PWV showed a gradual growth trend significantly,P < 0.05 was statistically significant.5,CKD patients group and healthy control group PWV difference,CKD group PWV is higher than the healthy control group,P < 0.05,has statistical significance. Conclusion: 1.In 1 and CKD patients, the PWV in hypertensive patients was higher than that in non hypertensive patients and healthy controlsThe onset of atherosclerosis is early.2.with the aggravation of kidney injury,PWV also showed a significant increasing trend, and PWV is an effective predictor of atherosclerosis in CKD patients.3.The arterial stiffness in the CKD group was higher than that in the healthy control group, indicating that the risk of cardiovascular disease was higher than that of the healthy population.
