A Comparative Study Of Two Type Internal Drainage For Pancreatic Pseudocyst

Objective: In this study,the clinical effect of two kinds of internal drainage of pancreatic pseudocyst(PPC)and the advantages and disadvantages of these two kinds of internal drainage were analyzed in our hospital.Materials and methods: Fifty six patients with PPC were divided into cyst jejunum Roux-en-Y anastomosis group(group A=35 cases)and cyst stomach anastomosis group(group B=21 cases)in the First Affiliated Hospital of Lanzhou University from March 2011 to July 2015.The general information,treatment methods,intraoperative blood loss,postoperative complications,postoperative ventilation time,eating time,hospitalization time,recurrence(follow-up 6-18 months)and other data.The advantages and disadvantages of the two groups were compared.Results: The operation time of group A(2.96± 0.13h),group B(1.96 ± 0.11 h,P<0.05);the amount of bleeding in the group A(361.57± 8.97ml),group B(292.38 ± 18.88 ml,P<0.05);postoperative hemorrhage of digestive tract in 2 cases of group A(5.7%),6 cases of group B(28.6%),a statistically significant difference(P<0.05);There was no recurrence in A group,1cases recurred in group B(n = 4.8%),there was no significant difference between the two groups(P>0.05);infection rates of postoperative pancreatic fistula in A group were 5.7% and 0%(2/35),group B were 0% and 4.8%(1/21),no statistically significant difference between the two groups(P>0.05);postoperative ventilation time,eating time,postoperative hospitalization time in group A were 3.54±0.65 d,4.74±0.74 d,6.60±0.65 d,group B were 3.71 ± 0.78 d,4.44± 0.87 d,6.38±0.80 d,no significant difference between two groups(P>0.05).Conclusion: Cystogastrostomy is a simple and fleeting operation,with less bleeding,but the postoperative digestive tract hemorrhage is more common;Roux-en-Y cystojejunostomy takes long operation time,intraoperative amount of bleeding,but gastrointestinal bleeding is rare,and the cyst location has no special requirements.Both surgical methods have their own advantages and disadvantages,so we should choose the surgical methods for different cases.
