| Objective:Through the analysis of cases of patients with pancreatic pseudocyst,aims to improve the level of diagnosis and treatment of pancreatic pseudocyst.Methods: Retrospective analysis of gender,age,pseudocyst size and location,etiology,clinical manifestation,auxiliary examination and treatment of 76 patients with pancreatic pseudocyst from January 2010 to October 2015 in ****.Results: In 76 cases of patients with pancreatic pseudocyst,42 cases(55.3%)were males and 34 cases(44.7%)were females;The maximum age was 78 years old,the minimum age was 19 years old,the average age was(41.5±1.5)years.There were 47 cases(61.8%)appeared in the body and tail of pancreas pseudocyst,13 cases(17.1%)appeared in the head of pancreas,16 cases(21.1%)appeared in the tail of the pancreas.Pseudocyst maximum diameter was 18 cm,the minimum diameter was 3.2cm,the average diameter was(6.7 ± 0.8)cm.The etiology of PPC included 36 cases of acute pancreatitis(47.4%),10 cases of pancreatic trauma(13.1%);4 cases of pancreatic surgery(5.3%),unexplained in 3 patients(3.9%).The clinical manifestations of PPC: 14 abdominal pain in 66 cases(90.4%),abdominal distension in 48 cases(63.2%),nausea,37 cases(48.7%),vomiting in 32 cases(42.1%),jaundice in 14 cases(18.4%),chills,fever,21(27.6%),anorexia in 27 cases(35.5%),body weight decreased 33 cases(43.3%),peritonitis,4 cases(5.3%),abdominal package in 59 cases(77.6%)and asymptomatic in 7 cases(9.2%).76 cases of PPC were all check blood amylase examination,41 cases(53.9%)were significantly increased,32 cases of cystic fluid amylase examination,32 cases(100%)were significantly increased.53 cases of PPC were all check CEA and CA19-9 examination,all in the normal range.All 76 cases with PPC did B-mode ultrasonography and CT examination,B-mode ultrasonography rate was 80.3%(61/76),CT positive rate was 90.8%(69/76);32 cases of MRI examination,positive detection rate was 65.6%(21/32);45 cases did MRCP examination,positive detection rate was 86.7%(39/45);19 cases did ERCP examination,positive detection rate was 68.4%(13/19);8 cases of EUS examination,positive detection rate was 75%(6/8).76 cases of patients with PPC did Observation of treatment,2 cases were lost to follow-up,11 cases were followed up for 3-6 months,8 cases pseudocysts were disappeared,3 cases were Significantly reduced and asymptomatic.Conservative treatment of 24 cases,20 cases were cured;percutaneous catheter drainage in 6 cases were cured;Surgical treatment of 28 cases,in which the external drainage 3 cases were cured,internal drainage in 23 cases,22 cases were cured(5 cases of surgery of anastomosis of the cyst and the stomach were cured;cyst and duodenal anastomosis surgery in 2 cases were cured;cyst jejunum Roux-en-Y type anastomosis in 16 cases,cured 15 cases,one case of recurrence),Cyst resection in 2 cases,all cured;Under endoscopic ultrasonography by gastric drainage in 5 cases,all cured.Conclusion:(1)Pancreatic pseudocyst has no gender differences.(2)Pancreatic pseudocyst is more common in middle-aged(30-50)age.(3)Pancreatic pseudocyst occurred in the body and tail of pancreas.(4)The most common disease of pancreatic pseudocyst is acute pancreatitis.(5)The clinical manifestations of patients with pancreatic pseudocyst is nonspecific,diagnosis is mainly based on imaging examination.(6)B-mode ultrasonography can be used as the preferred method for diagnosis of pancreatic pseudocyst.CT examination is the most accurate method for the diagnosis of pancreatic pseudocyst.(7)Pancreatic pseudocyst treatment depends on many factors like pseudocyst size,location,and underlying disease and other complications,should adopt the principle of individuation. |
