Objective: To retrospectively investigate the risk factors of affecting the prognosis of acute kidney disease in the hospitalized patients with chronic kidney disease;To investigate acute-on-Chronic Kidney Disease(ACKD)patients outcomes and whether the acute-on-chronic kidney disease is the independent risk factors of affecting outcomes.Methods: 346 chronic kidney disease patients from department of nephrology in Lanzhou University Second Hospital were included in this retrospective analysis.In this study,60 cases of chronic kidney disease(CKD)with acute renal injury(AKI),286 cases of chronic kidney disease patients.All patients were followed,patient's renal function was recorded and the prognosis of patient was systematically evaluated.Statistical Product and Service Solution(SPSS)21.0software was used for statistical analysis,the chi-square test(X2)was used for Count data analysis,t test of two independent samples for statistical analysis of metrological data between two groups,and independent risk factors for chronic kidney disease with acute renal injury were analyzed by multivariate logistic regression analysis.Survival analysis was performed using the Kaplan-Meier method,and the survival rate between the two groups was assessed using the Log-rank test.Results: A total of 60 of patients of ACKD were included in this study,which was 5.0% of hospitalized patients in the same period.Basic kidney disease history of nephrotic syndrome,hypertensive nephropathy,diabetic nephropathy is prone to chronic kidney disease with acute kidney injury.The average follow-up time was 33.7±17.8 months,Follow-up of patients effectively,254 cases of chronic kidney disease,chronic kidney disease with acute kidney injury group of 59 cases.During the entire follow-up,simple chronic kidney disease group mortality was2.8%,the mortality of chronic kidney disease with acute kidney injury group was 20.3%,the survival rate decreased in chronic kidney disease with acute kidney injury group compared with non-AKI group(X2=27.7,P=0.001).Multivariate logistic regression analysis showed that systolic blood pressure ?140 mmHg,diastolic blood pressure ? 90 mm Hg,history of diabetes,hypertension,prophylaxis using prophylaxis are independent risk factors in chronic kidney disease patients with acute Renal injury(the OR values were 1.1,0.8,2.5,1.4,2.8,P values were less than0.050).Conclusions: Basic kidney disease history of nephrotic syndrome,hypertensive nephropathy,diabetic nephropathy is prone to chronic kidney disease with acute kidney injury.Blood pressure?140 mmHg,diastolic blood pressure ? 90 mmHg,History of diabetes,hypertension,prophylaxis using prophylaxis are independent risk factors in chronic kidney disease patients with acute Renal injury.Compared with CKD patients,the mortality rate of patients with ACKD increased,but the prognosis of kidney and CKD patients were not statistically significant.
