Real-time Three-dimensional Echocardiography In Evaluation Of Curative Effect For Patients With Left Ventricular Aneurysm After Acute Percutaneous Coronary Intervention

Objective: Through the use of real-time three-dimensional echocardiography(RT-3DE),compares preoperative heart function indexs with the ones postoperative 2 months of patients with left venricular aneurysm(LVA)who undergo emergency percutaneous coronary interention menstruation(PCI),and then access PCI treatment effect.Methods: Apply RT-3DE,preoperative and postoperative 2 months related indicators of left ventricular function of 31 cases of patients with anterior wall myocardial infarction(AMI)and left ventricular aneurysm(LVA)were collected,including left ventricular end-diastolic volume(LVEDV),end-systolic volume(ESV),cardiac output(CO),left ventricular ejection fraction(LVEF)and left ventricular spherical index(SPI).According to the formula: body surface area(BSA(m2)=0.0061 x height(cm)+ 0.0128 x weight(kg),BSA),left ventricular end-diastolic volume index(LVEDVI),end systolic volume Index(ESVI),cardiac index(CI)were calculated(EDVI=EDV/BSA,ESVI=ESV/BSA,CI=CO/BSA),Objects were divided into three groups:functional LVA(12 cases),anatomical LVA(11 cases)and thrombotic LVA(8 cases).Cardiac function parameters before operation and of 2 months after operation were compared within a group and among the groups,and related anaysis were conducted on SPI,LVEF and cardiac function indicators before operation and of 2 months after operation.Results:(1)In the group of functional LVA,LVEF increased and SPI decreased two months after operation.The differences are statistically significant(P <0.05).(2)In the group of anatomical LVA,the differences of CO,CI,LVEF and SPI before and two months after operation are statistically significant(P<0.05).CO,CI,LVEF two months after operation are higher than preoperative ones,SPI decreased after operation,and the difference of SPI and LVEF are more obvious(P <0.01).(3)In group of thrombotic LVA,SPI decreased two months after operation,and the difference was statistically significant(P <0.05).LVEF increased two months after operation but the difference was not statistically significant(P >0.05).(4)Compared within each group,LVEDV,ESV,EDVI,ESVI of 2 months after operation all decreased.but the differences have no statistical significances(P>0.05).(5)LVEF and SPI differences before operation and of months after operation among 3 groups have no statistical significanc(P>0.05).(6)SPI was negatively correlated with LVEFand was positively correlate with LVEDV,ESV,EDVI,ESVI,CO CI before operation and two months after operation.LVEF was negatively correlated with SPI,LVEDV,ESV,EDVI,ESVI,CO,CI before operation and two months after operation.The correlation of LVEDV,ESV,EDVI,ESVI(P<0.01),CO,CI(P>0.05)have statistical significance.Conclusions:(1)Functional and anatomical LVA patients have improved in left ventricular morphology and its global systolic function two months after PCI operation,so operation is effecti ve;LVA patients with thrombosis have no obvious curative effect two months after PCI operation.(2)3RT-DE is a kind of effective method to evaluate short-term curative emergency PCI in patients with LVA,and SPI and LVEF are the two important indicators.
