0bjective: Using real-time three dimensional echocardiography (RT-3DE) to assess left ventricular volumes, function and mass in patients with essential hypertension.Methods: Left ventricular ejection fraction and left ventricular mass were determined by RT-3DE, MME and Two-Dimensional Echocardiography in 30 normal controls and 72 patients with essential hypertension which was divided into left ventricular hypertrophy(LVH) group and non-left ventricular hypertrophy(NLVH) group. DTI data which includes Ea, Aa and Ea/Aa were acquired from septum and lateral mitral anulus on apical four-chamber views.Result: (1)The satisfied RT-3DE images were acquired in 63 patients of 72 patients. (2)In both normal group and NLVH group, no statistical differences were found between RT-3DE and Simpson′s method in LVEDV, LVESV and LVEF calculations(P>0.05). (3)In LVH group, LVEDV and LVESV obtained by RT-3DE were greater than those obtained with two-dimensional echo(2DE), while LVEF obtained by RT-3DE was smaller than that obtained with two dimensional echocardiography (P<0.05). (4)The LVMI obtained by RT-3DE was greater than that obtained with two dimensional echocardiography, while smaller than that by M-mode Echocardiography (P<0.05). (5)Among three groups, LVEDV and LVESV measured by RT-3DE correlated with those measured by Simpson′s method(r=0.652~0.956, P<0.05). But the correlation coefficients of LVH group were lower than those of normal and NLVH group(P<0.05). (6)Compared with control group and NLVH group, LVEDV and LVESV measured by RT-3DE group increased in LVH group(P<0.05) and LVEF decreased (P<0.05). (7)Compared with control group, LVH group and NLVH group had lower early diastolic peak velocity(Ea)and, higher after diastolic peak velocity(Aa)and decreased Ea to Aa ratio(Ea/Aa) (P<0.05). (8)LVMI tended to increase while blood pressure increased in three groups of different hypertension grades.Conclusion: The application of real-time three-dimensional echocardiography presents an important tool for the quantification of left cardiac performance and left ventricular mass in the patients with essential hypertension.
