Tic disorder(TD)is a neuropsychiatric disorder involving multiple motor and phonic tics,which usually begin between the ages of 6 and 8,are sudden,rapid,stereotyped,and apparently purposeless movements or sounds that involve discrete muscle groups.Its prevalence has been established in numerous international community studies at around 1% of school children.The known contributory aetiological factors include complex genetics with possible contributions from the premature and perinatal environment and neuro-immunological mechanisms.Some of tic can be very severe to the point of causing pain to the patient.Apart from the physical consequences incurred by TD,tics can diminish patients' quality of life,social function,and even lifetime achievements.In recent years,an increased prevalence in TD has been reported.Drugs useful for TD include neuroleptics such as aripiprazole,tiapride,risperidone and haloperidol,which result in metabolic adverse effects frequently,like excessive sleepiness,adiposity,extra pyramidal symptoms.Moreover,a trial of treatment with these drugs is long(around one year)and part of patients with TD suffer relapse during withdrawal period.Traditional Chinese Medicine(TCM)is safe and effective for patients with TD.However,few studies have examined long-term outcome to evaluate the recurrence rate after a trial of TCM treatment.In this study,a TCM formula,called Varied Chaihu Shugan San was used in patient with TD,compared with western medicine group.Both groups were followed for one year.Objective: To study the efficacy and safety of Varied Chaihu Shugan San on children with TD.Methods: Using prospectie study method.160 cases of tic disorders meet the standard of traditional Chinese medicine(TCM)of liver depression and spleen-qi deficiencyand western diagnostic code of syndrome differentiation.According to parents' willing,tic disorders are divided into traditional Chinese medicine group with102 cases and western medicine group with 58 cases,traditional Chinese medicine group was given Varied Chaihu Shugan San treatment and western medicine group was given Aripiprazole tablets and Tiapride hydrochloride.There are 3 months for this treatment,observing the recently clinical efficacy of two groups,side effect and the long-term efficacy,the recurrence rate,treatment compliance in one year.Results: Traditional Chinese medicine group and western medicine group were effective in the treatment of tic disorders in children.The recently clinical efficacy of two groups : the total efficacy of 93.7% Chinese medicine group was obviously higher than that of western medicine group,the total efficacy of 82.4%,with significant statistical significance(P ? 0.05);The long-term efficacy : the total efficacy of 92.6% Chinese medicine group was obviously higher than that of western medicine group,the total efficacy of 66.7%,with significant statistical significance(P? 0.05).Side effect: 2.1% side effect of traditional Chinese medicine group was obviously lower than western medicine group 25.5%,with significant statistical significance(P ? 0.05).Recurrence rate: followed up for 1 year,and the recurrencerate of 5.3% of traditional Chinese medicine group was obviously lower than western medicine group of 17.6%,with significant statistical significance(P ? 0.05).Chinese medicine group whose the average time of taking medicine is(3.86 ± 2.00)months is less than western medicine whose time is(8.00 ± 2.28)months.The rate of lost to follow-up: followed up for 1 year,traditional Chinese medicine(TCM)lost to follow-up rate of 2.1% is lower than western medicine group of 9.8%,but no significant statistical significance(P ? 0.05),Chinese medicine compliance may be also higher than western medicine group.Conclusion: Varied Chaihu Shugan San is effective and safety with a lower recurrence rate,a shorter course of treatment and fewer adverse effects than western medicine on treating TD in children.
