A Study On ICU Patients Infected With Acinetobacter Baumannii In TangShan

Objectives To understand the features of integrated ICU hospital infection in Tangshan,The distribution of Acinetobacter baumannii infection in ICU patients is discussed by sex,age,time and diseases.The origin and drug resistance and classification of Acinetobacter baumannii is also analyzed.In order to take timely preventive measures to reduce the incidence of Acinetobacter baumannii infection in hospital ICU,and to guide the clinical effective treatment and provide theoretical basis for rational use of drugs.Methods Patients who were stay in Tangshang Workers' Hospital ICU,Kailuan Hospital ICU,North China University of Sceience and Technology Affiliated Hospital ICU,Tangshan Second Hospital ICU from 1 August 2015 to 31 July 2016 were choosen as the subject of the study.The clinical data of the patients were collected by self-made questionnaire according to patient cases.Database is established by epidata3.0,and analysised by SPSS software,to understand the clinical characteristics and drug resistance of Acinetobacter baumannii infection.At the same time,the Acinetobacter baumannii strains isolated from ICU were collected,then DNA of Acinetobacter baumannii was extracted,and the bacteria type were analyzed by RAPD.Results 1 The infection rate of Acinetobacter baumannii was 5.6% in Tangshan urban ICU.The incidence of Acinetobacter baumannii infection in ICU of Tangshang Workers' Hospital was 5.7%.The incidence of Acinetobacter baumannii infection in ICU of Kailuan Hospital was 5.6%.The incidence of Acinetobacter baumannii infection in ICU of Tangshan Second Hospital was 2.9%.The incidence of Acinetobacter baumannii infection in ICU of North China University of Sceience and Technology Affiliated Hospital was 6.7%.Differences between them were statistically significant(P<0.05),and the infection rates of Acinetobacter baumannii in different hospitals were different.2 The incidence of Acinetobacter baumannii infection of ICU patients in Tangshan was 3.6% in the first quarter,6.3% in the second quarter,6.9% in the third quarter and 3.2% in the fourth quarter.The difference was statistically significant(P<0.05).The incidence of Acinetobacter baumannii infection in ICU during summer and fall was higher than that during spring and winter.3 The incidence of Acinetobacter baumannii infection of male patients in ICU of Tangshan hospital was 6.7% and that of female patients was 4.1%.The difference was statistically significant(P<0.05).The incidence of Acinetobacter baumannii infection of male patients in ICU was higher than that of females patients.4 The incidence of Acinetobacter baumannii infection in 10 years old group patients was 0.0%,20 years old group patients was 1.9%,30 years old group patients was 2.0%,40 years old group patients was 2.4%,50 years old group patients was 5.7%,60 years old group patients was 6.3%,70 years old group patients was 8.3%,80 years old group patients was 6.6%,90 years old or older group patients was 6.1%.The infection rate of Acinetobacter baumannii in different age groups was different(P<0.05).The infection rate of high age group(>60 years old)was higher than that of low age group(<60 years old).5 The incidence of Acinetobacter baumannii infection of patients who stay in ICU with compound trauma was the highest,the rate was 26.1%,the incidence of digestive disease patients was 12.3%,followed.6 Acinetobacter baumannii resistance to aminoglycosides,Carbapenems,quinolones and other clinical commonly used drugs was more than 80%.Drug resistance of acinetobacter baumannii is serious,but Acinetobacter baumannii is highly sensitive to polymyxin.7 After Random Amplified Polymorphic DNA,154 strains of Acinetobacter baumannii were divided into 9 type that marked as A,B,C,D,E,F,G,H,I.B type and C type as the main bacteria type.The major bacteria type were distributed among each hospital.The spread is possible among different hospitals in Tangshan.Conclusions 1 The incidence of Acinetobacter baumannii infection in ICU was different in different hospitals in Tangshan.The incidence of Acinetobacter baumannii infection was higher in males than that in females,the incidence during summer and autumn season is generally high.Patients with complex trauma and surgery have a high incidence.2 Acinetobacter baumannii strains were mainly isolated from the sputum in ICU,Its resistance is higher than Pseudomonas aeruginosa,Acinetobacter baumannii drug resistance phenomenon is common and serious.The major bacteria type were B type and C type.
