The Relationship Between Health Literacy And The Status Of The Disease Uncertainty With The Young And Middle-aged Patients With Coronary Heart Disease

Objectives To study the status of health literacy and disease uncertainty and analysis the factors which influence them among young and middle-aged patients with coronary heart disease(CHD).Next,to analysis the influencing factors and the correlation between health literacy and disease uncertainty,in order to provide a theoretical basis on enhancing the health literacy issues,decreasing the uncertainty in illness and improving the life quantity of young and middle-aged patients with CHD.Methods This project adopts cross-sectional study to analysis 500 young and middleaged patients with CHD hospitalized in Cardiovascular Department of Tangshan People's Hospital from 2015.10 to 2016.10.Patients are surveyed 1~3 days after hospitalization by the unified scales of chronic health literacy,disease uncertainty of Mishel,the questionnaires of coronary heart disease,self-rating anxiety,as well as the self-efficacy for chronic disease scales and general conditions(patient with stable condition).Face to face interview is used to teach the patients how to fill in the questionnaires on the spot.When the questionnaires are finished,check and take back them after confirmation.Then the parameters of general demographic and clinical features of them are described by frequency,percentage,mean ± standard.Single factor influences health literacy and disease uncertainty of young and middle-aged patients with CHD is analyzed by Student t test and One-way ANOVA.Multiple factors influence them are analyzed by multiple Logistic regression.Furthermore,the correlation between health literacy and disease uncertainty of young and middle-aged patients with CHD is analyzed by Spearman Correlation.Results 1 The percentage of young and middle-aged patients with CHD with low healthy literacy is 40% among 500 patients.The percentages of young and middle-aged patients with CHD who has middle and high level of disease uncertainty are 89.6% and 0.4%,respectively.2 The healthy literacy of young and middle-aged patients with CHD is in good level with total score of 97.77±15.452.Among each parameter,the score of capability for acquiring information is the optimum with score of 4.40±0.752 and the intention of improve health is the worst one with score of 3.64±1.219.The results of multiple Logistic Regression demonstrate that degree of education,knowledge of CHD,self-efficacy for chronic disease,monthly income,residence and anxiety are all factors which influence health literacy for young and middle-aged patients with CHD.3 The disease uncertainty of young and middle-aged patients with CHD is in middle level with total score of 89.82±12.280.Among each parameter,the score of ambiguity is the highest with score of 2.97±0.675 and the complexity is the lowest with score of 2.32±0.512.The results of Multiple Logistic regression suggest that self-efficacy for chronic disease,anxious levels,monthly income,age and knowledge of CHD are all factors influence the disease uncertainty.4 These scores of health literacy and disease uncertainty with complexity among young and middle-aged patients with CHD are negatively correlated with,respectively(r=-0.368,r=-0.312,P<0.001).While the disease uncertainty is negatively correlated with the capability of acquiring information of health literacy(r=-0.318,P<0.001).Conclusions 1 The healthy literacy of young and middle-aged patients with CHD is in good level which are influenced by the degree of education,knowledge of CHD,selfefficacy for chronic disease,monthly income,residence and anxiety.2 The disease uncertainty of young and middle-aged patients with CHD is in middle level which isinfluenced by the self-efficacy for chronic disease,anxious degree,monthly income,age and knowledge of CHD.3 There is negative correlation between the total score of health literacy and the complexity dimension among young and middle-aged patients with CHD,so as the total score of disease uncertainty.The capability of acquiring information is negatively correlated with disease uncertainty in health literacy.
