The Value Of Xanthine Oxidase Diagnosis In Patients With Chronic Systolic Heart Failure,and Its Relationship With Bnp And Troponin T

Objective: The study of serum xanthine oxidase(XO)in patients with chronic systolic heart failure is used to explore the the relationship between B type natriuretic peptide(BNP),T(Troponin T,troponin c Tn T),and evaluate the diagnostic value of XO in patients with chronic systolic heart failure,in order to provide a new diagnostic criteria for chronic systolic heart failure.Methods: Choose from 2015-07 to 2016-07,according to the "China heart failure guidelines for the diagnosis and treatment of 2014 patients with HE-REF in hospital",in the Department of Cardiology of the Xinjiang Uygur Autonomous Region people's Hospital,94 cases of patients who met the HE-REF diagnostic criteria as the case group.During the same period,90 healthy persons were selected as control group.Exclude with chronic systolic heart failure diagnostic criteria,renal insufficiency,gout,chronic obstructive pulmonary disease,cardiac or vascular tumor or congenital deformity,new onset myocardial infarction(<1 months),blood system diseases,pregnancy,systemic lupus erythematosus disease.Enzyme linked immunosorbent assay was used to detect the activity of XO.Double antibody sandwich enzyme-linked immunosorbent assay(ELISA)was used to detect CTn T.BNP level was measured by(Biosite company)Bo Triage bedside rapid quantitative myocardial infarction heart failure diagnostic instrument.The value of LVEF was determined by using the color Doppler diagnostic instrument with VIVIDT frequency conversion probe of GE 2MHz-4MHz.The baseline data of age,height,weight and blood lipid of the two groups were collected.To investigate the relationship between XO and BNP,c Tn T,calculate the xanthine oxidase diagnostic test sensitivity,specificity and Youden index,receiveroperating curve(ROC curve),so as to better evaluate the diagnostic value of diagnostic test.Results:(1)There was no significant difference in age,weight,height and BMI between the two groups;(2)XO was positively correlated with BNP,and the degree of correlation was strong,and CTn T was positively correlated,but the correlation was weak;(3)The area under the ROC curve of XO in the diagnosis of chronic systolic heart failure(AUGROC=0.791),it is suggested that the diagnostic criteria have certain accuracy,and it is of high diagnostic value in clinical application;(4)This experiment is based on the Youden index(correct diagnostic index)that XO diagnosis of ideal critical point for chronic systolic heart failure is 3633.1pg/ml,its sensitivity was 0.734,specificity 0.733,positive predictive value and negative predictive value of 0.74 and 0.73.Conclusion: Serum xanthine oxidase in patients with chronic systolic heart failure(XO)and B type natriuretic peptide(BNP),troponin T(c Tn T)was positively correlated.Serum xanthine oxidase(XO)in the diagnosis of chronic congestive heart failure was secondary value,the best critical point for the diagnosis is 3633.1pg/ml.
