ObjectivesChronic heart failure(CHF)is a complex syndrome including hemodynamic, neurohumoral and metabolic abnormalities,which is a severity stage of many cardiac disease.How to diagnose CHF and assess the cardiac function level is related to the treatment and prognosis of CHF.Adiponectin is an adipocyte-specific cytokine, abundant in plasma and has important metabolic effects,such as reducing plasma glucose,increasing insulin sensitivity,antiinflammatory,anti-atherosclerosis,and so on. Recent foreign studies showed that adiponectin may take an important role in the pathogenesis of CHF for high plasma adiponectin levels were found in patients with Chronic systolic heart failure.Similarly,many studies showed that plasma brain natriuretic peptide(BNP)levels are important for diagnosis and prognostic predictor of CHF.We have investigated the plasma levels of adiponectin and BNP in Chinese patients with systolic CHF,and the association with cardiac function.Materials and MethodsThe study consisted of 60 inpatients diagnosed with systolic CHF and 30 age-, gender-and body mass index(BMI)-matched healthy controls who visited shengjing hospital of China medical university from January 2007 to September 2007.Diagnosis of systolic CHF was based on history of cardiac disease,symptom,physical sign and left ventricular ejection fraction(LVEF)≤40%in ultrasonic cardiogram evaluation.All patients were divided into three subgroup base on The New York Heart Association (NYHA)functional classⅡ-Ⅳ.The plasma level of adiponectin and BNP was measured by sandwich ELISA and MEIA method,respectively,in all subjects.Comparisons of the plasma levels of adiponectin and BNP among the patients groups and controls group were conducted by student's t-test and one-way ANOVA of variance.Pearson's correlation coefficient method was used for linear correlation analyses of adiponectin level and BNP level in patients with systolic CHF.A value of P<0.05 was considered significant.All data analyses were performed using SPSS 13.0.ResultsThe plasma levels of adiponectin(10.86±7.96 mg/L)and BNP(1318.73±1237.91 pg/ml)in patients group were significantly higher compared with those in controls group(1.42±0.56 mg/L)(P<0.01)(61.63±13.95pg/ml)(P<0.01),respectively.The plasma levels of adiponectin in NYHA function classⅡtoⅣsubgroup in patients group(classⅡ4.87±2.63mg/L,classⅢ11.04±8.19 mg/L,classⅣ16.67±7.47 mg/L) were significantly higher than that in controls group(1.42±0.56 mg/L),respectively. Moreover,the plasma adiponectin level increased with the severity of NYHA functional class(P<0.05).The plasma levels of BNP in NYHA function classⅡtoⅣsubgroup in patients group(classⅡ343.18±143.93pg/ml,classⅢ1173.41±1088.08 pg/ml,classⅣ2502.71±1180.96pg/ml)were respectively significantly higher than that in controls group(61.63±13.95 pg/ml),and the plasma BNP level increased with the severity of NYHA functional class(P<0.05).A significant positive correlation was observed between the plasma level of adiponectin and BNP in systolic CHF patients group(r=0.65,P<0.01).ConclusionThe plasma levels of adiponectin and BNP in patients with systolic CHF were significantly higher than those in healthy controls respectively,the plasma levels of adiponectin and BNP increased with the severity of NYHA functional class and there was a significant positive correlation between plasma adiponectin level and BNP group.
