ObjectivesAs frailty being one of the most important geriatric syndromes,the quality of life and the survival time of the elderly over the age of 65 are seriously affected.The causes of frailty of different groups are not all the same.This paper aims to survey the frailty status of dialysis patients in our dialysis center,and evaluate the influencing factors of hemodialysis patients with frailty,improve the etiology investigation of hemodialysis patients,and strengthen the prevention for frail in dialysis patients,quality of life and help to improve the prognosis of hemodialysis patients.MethodsBy convenient sampling method to collect the clinical data of 85 cases of hemodialysis patients in hemodialysis room,whose data are relatively complete,in the Department of Nephrology Beijing Chaoyang Hospital affiliated to the Capital Medical University on 2016.4-5.Using the FRAIL questionnaire for frailty assessment,which is a simple and suitable method,especially for the bedside.It includes screening 5 aspects of fatigue,endurance,balance,diseases,weight loss(See Appendix 1).The FRAIL score of 0 is less than or equal to 2 is divided into A group,more than 3 were divided into B group.Single factor analysis was used to compare the data of 2 groups of patients and the results of auxiliary examinations.Logistic regression analysis was performed on the independent variables with statistical difference.Combined with the general definition of frailty,FRAIL 0 is divided into nonfrail,1-2 is divided into prefrail,and more than 3 points for the frail.Through the above methods to analyze the influence factors of frailty.Results1.The mean age of 85 hemodialysis patients was(60.73 ±13.45)years old,of which there were male(n=51)(60%)and female(n = 34)(40%).The mean age of dialysis was(5.80 ± 4.25)years,BMI(23.55 ±3.59)kg/m2,CCI(2.8 +±0.81).A history of falls within 1 years is 17 cases(20%),50 cases(58.8%)get exercise habits,78 patients(91.8%)with hypertension,26 cases(30.6%)of patients with coronary heart disease,25 cases of patients with diabetes mellitus(29.4%),20 cases(23.5%)have hyperlipemia.2.According to the FRAIL,patients whose score is 2 or less were divided into A group,score greater than 3 were divided into B group.A group and B group in age(59.71 ±13.51 vs 67.92 ± 12.53,P=0.045),grip strength(23.89± 10.10 vs 14.00 ±5.72,P=0.001),CCI score(mean rank 40.53 vs average rank 56.65,P=0.019),exercise(66.7% vs 15.4%,P=0.001),patients with coronary heart disease(69.2% vs 23.6%,P=0.003),there were significant difference(P<0.05).There was a more significant difference in grip strength,exercise,and coronary heart disease(P<0.01).3.According to the frail stage,we have a new grouping.That is,FRAIL 0 is divided into nofrail,1-2 is divided into prefrail,and more than 3 points for the frail group.There were 68 cases in the pre-and the frail group(80%).The 3 groups in the grip of strength(27.35±7.29 vs 22.81±10.64 vs 14.00± 5.72),prealbumin(0.37±0.08 vs 0.32±0.07 vs 0.28±0.05),the constituent ratio of exercise(58.8% vs 69.1% vs 15.4%),the proportion of coronary heart disease(6% vs 29.1% vs 69.2%),CCI score,there are significant differences between the 3 groups(P<0.05).4.By stepwise regression Logistic analysis,the regression coefficient of the equation was grip strength(?=-0.606,P<0.05),coronary heart disease(?=2.097,P<0.05),exercise(?=-2.359).Coronary heart disease(OR=8.14)is a risk factor for frail,and increased grip strength(OR=0.546),exercise(OR=0.095)is a protective factor.ConclusionsFrailty is common in hemodialysis patients.Coronary heart disease,no exercise,low prealbumin are risk factors for patients with hemodialysis.
