?Objective?To investigate the risk factors of high grade pivot shift ACL injury,and to provide the basis and reference for improving the prognosis of patients with ACL injury.?Methods?a retrospective review from 2014 04 to October 2015 admitted to the Department of orthopedics,401 Hospital of PLA by strict clinical diagnosis of patients with anterior cruciate ligament injury in 60 cases,were randomly divided into normal group,mild group,high axial shift axial shift group,20 cases in each group.The patients underwent ACL reconstruction treatment,perfect preoperative examination,3 groups were collected the basic information(including gender and age of patients,injury to the operation time),MRI data(including PITS,PITS measurement and medial lateral ALL abnormalities)of lateral meniscus and medial meniscus injury and during the operation of application of SPSS 17.0 data acquisition and processing software for univariate and multivariate Logistic regression analysis,risk analysis of high axial shift of the anterior cruciate ligament injury factors.?Results?In the group with high grade pivot shift,lateral posterior-inferior tibial slope was slightly higher than the group with low grade pivot shift and normal group(P<0.05group);In the group with low grade pivot shift,lateral posterior-inferior tibial slope was slightly higher than the normal group(P<0.05);In three groups,single factor analysis showed that anterior cruciate ligament injury with high grade pivot shift incidence with sex,age,the time from injury to operation,medial posterior-inferior tibial slope,medial meniscus injury showed no significant difference(P>0.05);anterior cruciate ligament injury with high grade pivot shift is closely relationship between the incidence of lateral posterior-inferior tibial slope,anterior lateral ligament injury,and lateral meniscal injury(P<0.05);Logistic regression analysis showed that anterior cruciate ligament injury with high grade pivot shift and the incidence rate of lateral posterior-inferior tibial slope,anterior lateral ligament injury,and lateral meniscal injury(P<0.05)?Conclusion?(1)the risk factors of anterior cruciate ligament injury with high grade pivot shift were larger lateral posterior-inferior tibial slope,anterior lateral ligament injury,and combined lateral meniscal injury;(2)according to the risk factors in the treatment of the anterior cruciate ligament injury with high grade pivot shift in simple anterior cruciate ligament reconstruction,should also correct the lateral posterior-inferior tibial slope,suture repair of the meniscus,anterior lateral ligament at the same time to strengthen or reconstruction,it is helpful to improve the therapeutic effect,improve the prognosis.Reduce the incidence of complications.
