Purpose To evaluate left ventricular global longitudinal strain in endocardium,middle myocardium and epicardium of patients with breast carcinoma treated with anthracyclines by two-dimensional layered strain technology.investigate the value of this technology in evaluation of the early changes of left ventricular systolic function in breast carcinoma patients treated with anthracyclines.To guide clinical rational using drug and improve patients' quality of life.Methods The study included 20 patients after surging without chemotherapy as the contral group and 32 breast cancer patients who were treated with anthracycline-based chemotherapy.Chemotherapy cycle for 3 months and 5 months images were captured by transthoracic echocardiographic before and after chemotherapy.Global longitudinal strain(GLS)and each segment strain in 3myocardial layers(endocardium,middle myocardium and epicardium)was measured by Echo PAC soteware.Measuring mitral valve mouth early diastolic peak flow velocity(E),late diastolic peak flow velocity(A),under the condition of TDI measuring mitral annulus lateral ventricular septal early diastolic peak flow velocity(E),late diastolic velocity(A),calculation E/e ratio.Then,Comparing the results of different groups.Results1 Patients after chemotherapy compared with control group,E/E ratio significantly increase,the difference was statistically significant(p< 0.05),but left ventricular ejection fraction(LVEF)had no statistical difference(P>0.05).2 Compared with contral group,as the extension of chemotherapy cycle,GLS of 3myocardial layers were reduced in breast carcinoma patients treated with anthracyclines(P<0.05).3 Compared with contral group,each layer of each segment of longitudinal strain decreased in breast carcinoma patients treated with anthracyclines,in addition to the apex section(P<0.05).4 As the extension of chemotherapy cycle,the outer membrane of each segment and middle basal segment strain values to reduce,the difference wasstatistically significant(p < 0.05)Conclusion1.longitudinal layer-specific strain technology can accurate assessment of left ventricular of 3 myocardial layers whole and local systolic function,and it can provide a new method for judging myocardial damage.2.Chemotherapy drugs affect breast carcinoma patients left heart systolic function and diastolic function.
