| Objective: To explore the relationship between blood lipids、serum Cystatin C(CysC)、homocysteine(Hcy)and Parkinson’s disease,and to explore the relationship between these indexes and the movement subtypes,age of onset subtypes and severity of disease,and discuss its relevance to age,course of disease and Levodopa dosage.Methods: 161 patients with Parkinson’s disease were selected as the case group,107 healthy control as the control group.The General information,MMSE score,Serum total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),apolipoproteinA1(ApoA1),apolipoprotein B(ApoB),lipoprotein a(Lpa)and Cys C,Hcy levels were compared between different groups;Logistic regression analysis was used to study the risk factors of Parkinson’s disease;Patients were grouped according to the Hoehr-Yahr classification,main movement symptoms and the age of onset respectively.The indexes above were compared in different groups,different subtypes;The indexes that were different between case group and control group were used to analyze it’s correlation with age,course of disease and levodopa dosage by the Spearman method;The ROC curve was mapped to analyze the diagnostic value of above indexes for Parkinson ’s disease.Results: The levels of serum TC,TG and ApoB in the Parkinson’s disease group were lower than that in the control group,and the levels of HDL-C,Hcy and CysC were higher than that in the control group(P <0.05).The decreased serum ApoB levels and increased HDL-C,Hcy levels were independent risk factors for Parkinson’s disease(P <0.05).The level of CysC in early-onset patients was lower than that in late-onset patients(P <0.05).The serum Hcy(r=0.199,P<0.05),CysC(r=0.283,P<0.05)were positively correlated with age,TG was negatively correlated with age(r=-0.203,P<0.05)and course of disease(r=-0.199,P<0.05),UPDRSIII score were positively correlated with course of disease(r=0.436,P<0.05)and Levodopa dosage(r=0.424,P<0.05).The area under the ROC curve of serum ApoB for Parkinson’s disease was 0.341(P <0.05),which had no diagnostic value for Parkinson’s disease.The area under the ROC curve of serum HDL-C for Parkinson’s disease was 0.588(P <0.05),which had lower diagnostic value for Parkinson’s disease,the corresponding HDL-C boundary value was 1.135 mmol/L,the diagnostic senstivity was 60.2%,the Specificity was 53.3%;The area under the ROC curve of serum Hcy for Parkinson’s disease was 0.643(P <0.05),which had lower diagnostic value for Parkinson’s disease,the corresponding Hcy was 16.5μmol/L,the diagnostic sensitivity was 60.2% and the specificity was 65.4%.Conclusion: Low serum TC,TG,ApoB and high serum HDL-C,Hcy,CysC may be associated with the occurance and development of Parkinson’s disease;Decreased serum levels of ApoB and increased levels of serum HDL-C and Hcy were independent risk factors for Parkinson’s disease;Early-onset Parkinson’s disease and late-onset Parkinson’s disease may be different in the pathogenesis of disease. |
