| Objective : To investigate the charactristics and changes of deceleration capacity and deceleration runs in patients with chronic heart failure and study its clinical significance。Methods:we enrolled 145 patients with chronic heart failure and 276 people without heart disease as control grorup,all inductees underwent 24 h ambulatory electrocardiogram and ultrasonic echocardiography and other examination,we collect the basic information of the two groups,DC values and DRs、SDNN、HRT were calculated from the DMS software,the averaging heart rate,premature ventricular beats,recording left ventricular ejection fraction,N-terminal Pro-brain-type natriuretic peptide(NT-Pro BNP),serum potassium were also collected。Various indicators of the two groups were compared,the related indexes were compared in patients with different left ventricular ejection fraction,we also analysized the correlation between DC(DRs)and HRT、SDNN、LVEF and explain its clinical significance。Results:(1)compared with control group,DC value、TS and SDNN、LVEF was significantly decreased in the heart failure patients,while average heart rate and NT-Pro BNP increased,and the level of AC and TO value increased,the number of ventricular premature beats significantly increased,the differences were all statistically significant(p<0.05),but potassium in the two groups of patients have no statistically significant(p>0.05)。(2)However,there is no statistical significance between HfpEF,HfmrEF,HFrEF groups on the value of DC、AC、SDNN and mean heart rate、TS、potassium irons、NT-Pro BNP(p>0.05),besides,there is no correlation between DC and LVEF in heart failure patients。(3)Increasing with the level of stratification,SDNN decreased,mean heart rates and NT-Pro BNP increased,and these differences were statistical significant(p<0.05),but the indicators of LVEF and potassium,the TO,the TS did no change(p>0.05)。(4)DC value was highly correlated with SDNN and TS,NT-Pro BNP,the correlation were positive,was negatively correlated with average heart rate,these difference was statistical significant(p<0.05),nevertheless,there were no statistical significance between DC and LVEF or potassium。The correlation with DRs and these index are similar to DC。Conclusion:DC values significantly decreased in patients with chronic heart failure,which indicate that the vagus never function is impaired,and protective effect on cardiovascular system is reduced,DC can be used to assess the severity of illness and risk stratification,and there is no obvious correlation with LVEF,furthermore,combine DC and DRs,which can be used to evaluation and screen out high-risk patients,DC and DRs have quite better practical value for clinical guiding。... |
