Research On The Distribution And Dynamic Changes Of TCM Syndrome In Breast Cancer Chemotherapy Period

Objective ? Investigate the distribution and dynamic changes of the TCM syndrome in breast cancer chemotherapy period.? Explore the feasibility of cluster analysis and factor analysis in TCM syndrome research,and find the probable differences in the statistics.Methods A questionnaire is made from the scatting of the post articles,the investgation in advance and the suggestion from the experts.The questionnaire contains 17 clinical symptoms,which is used to collect the relative information at 4 points:at the beginning of the chemotherapy,after 2 chemotherapeutic cycles,after 4 chemotherapeutic cycles,and at the end of the chemotherapy.The information will be saved in a database.Then researcher will make the screening of the clinical symptoms through removing the ones which are at low rate of occurrecne(<5%).With the help of the experts' experience,the remaining symptoms are statistically classified by using the cluster analysis and factor analysis,simultaneously considering the tongue picture and pluse picture,so as to determine the TCM syndrome.Finally,discover the law of dynamic changes through the comparison of syndromes distribution among 4 stages.Results At the beginning of the chemotherapy,three TCM syndrome can be inferred by the experts through the 6 effective symptoms,and the statistical results seperately from cluster analysis and factor analysis are same.90 patients are divided into three clusters,15 ones belong to Spleen-Hypofunction-And-Dampness-Accumulation syndrome(16.67%),21 ones belong to Liver-Depression syndrome(23.33%),and 54 ones belong to Balance-of-Yin-and--Yang syndrome(31.11%).After 2 chemotherapeutic cycles,three TCM syndrome can be inferred by the experts through the 12 effective symptoms,and the statistical results seperately from cluster analysis and factor analysis are not the same.By the result from the factor analysis,90 patients are divided into three clusters,34 ones belong to Liver-Stagnation-And--Spleen-Deficiency syndrome(37.78%),28 ones belong to Heart-And-Liver-Fire syndrome(31.11%),and 28 ones belong to Balance-of-Yin-and-Yang syndrome(31.11%).By the result from the cluster analysis,90 patients are also divided into three clusters,34 ones belong to Spleen-Deficiency syndrome(37.78%),28 ones belong to Heart-And-Liver-Fire syndrome(31.11%),and 28 ones belong to Balance-of-Yin-and-Yang syndrome(31.11%).After 4 chemotherapeutic cycles,four TCM syndrome can be inferred by the experts through the 14 effective symptoms,and the statistical results seperately from cluster analysis and factor analysis are same.90 patients are divided into four clusters,35 ones belong to Spleen-Deficie--ncy syndrome(38.89%),20 ones belong to Heart-And-Liver-Fire syndrome(17.78%),28 ones belong to Yin-Deficiency-Inner-Heat syndrome(31.11%),and 11 ones belong to Balance-of-Yin-and-Yang syndrome(12.22%).At the end of the chemotherapy,four TCM syndrome can be inferred by the experts through the 14 effective symptoms.The statistical results seperately from cluster analysis and factor analysis are not exactly same due to the symptom of loose stool,although it does not cause the essential effect on the final result.90 patients are divided into four clusters,16 ones belong to Liver-Depression-And-Spleen-De--ficiency syndrome(17.78%),24 ones belong to Liver-Blood-Deficiency-And-Spleen-Deficie--ncy syndrome(26.67%),43ones belong to Yin-Deficiency-Inner-Heat syndrome(47.78%),and 7ones belong to Balance-of-Yin-and-Yang syndrome(7.78%).Conclution Deficiency syndrome becomes more and more frequent with the chemotherapy's going,such as qi deficiency,blood deficiency and yin deficiency,while excess syndrome such as qi stagnation,inner heat and phlegm dampness becomes rarer and rarer.There are Spleen-Deficiency syndrome and Liver-Stagnation syndrome at the beginning of the chemotherapy.After 2 chemotherapeutic cycles,there are(Liver-Stagnation-And-)Spleen-Deficiency and Heart-And-Liver-Fire syndrome.Spleen-Deficiency syndrome,Yin-D--eficiency-Inner-Heat syndrome and Heart-And-Liver-Fire syndrome are inferred after 4 chemotherapeutic cycles.At the end of the chemotherapy,Deficiency syndrome appears mostly frequent such as Yin-Deficiency syndrome.There are no essential difference between two statistical methods,both cluster analysis and factor analysis can be used in normalized research of TCM syndrome.
