The Passive Antibody Seroreversion Research Of Children Born To Mothers With Syphilis

Objective:To investigate syphilis passive antibody seroreversion discipline of children born to mothers with syphilis from STD clinic in our hospital,which provide the basis for the prevention and treatment of congenital syphilis.Methods:The objects of this study were the STD clinic normal children born to mothers with syphilis during the period from January 2010 to December 2016 in the First Affiliated Hospital of Kunming Medical University.Collecting mother and their children epidemiological characteristics and clinical features,children' serum of TPPA and TRUST titer were measured at 3 months,6 months,9 months,12 months,15 months and 18 months of age.The mother's serum of TPPA and TRUST was measured before and after treatment,and 18 months after the age TPPA is not negative,the children was diagnosed as congenital syphilis,included in the positive control group.The data were input to the Excel database and analysised the related factors affecting time of seroreversion of syphilis passive antibody of children born to mothers with syphilis with complete follow-up data by SPSS 17.0 statistical software.Results:A total of 33 cases of complete follow-up data of Syphilis passive antibody positive children and their mothers,6 cases of congenital syphilis and 5 cases of their mothers.1 case is the adoption of children,the mother is unknown.(1)Of 33 cases of syphilis passive antibody children boys were 15 cases while girls were 18 cases,boys to girls ratio was 5:6;Han people cases accounted for 93.9%(31/33).There are congenital syphilis children 6cases,girls were 5 cases while boys were 1case,age were 1.5 to 11 years old,everyone was Han people.(2)31 cases of pregnant women with syphilis,of which 2 were twin mothers,the production age were 19 to 38 years old,the average age of 29.5 years old;Congenital syphilis children' morthers age were 20 to 27 years old.Most were Han pepole and were married.Each education level and each occupation were distributed.All of cases were transmissed by heterosexual sex.38.71%of mother with syphilis main cause of treatment was physical examination,followed by the pregnancy test(35.5%),other pregnancy examination(22.6%),adopted for 1 case(3.2%).Of 31 cases latent syphilis were 29 cases(93.55%%),secondary syphilis were 2 cases(6.5%).Combined with genital herpes in 1 case.Congenital syphilis children' morther 1 case(20.0%)of 5 cases was found by physical examination,other 4 cases were by the pregnancy test(80.0%).HIV test were negative,and all of cases treatment with benzathine penicillin.(3)6 cases of congenital syphilis in children,the age was 1.5 to 11 years old,above were latent syphilis.Of which 5 cases were treated by benzathine penicillin,1 case was ceftriaxone sodium.(4)Syphilis passive antibody children's TRUST negative rate was of 100%before 12 months ago,while TPPA before 18 months.The lower the TPPA initial titer,the shorter seroreversion time required for,the average monthly decline of 2 times.The TPPA initial titer of All object group,1:1280 group,1:640 group,and 1:320 group,TPPA decreased by 3 times in 3 months of age,and was completely negative in 18 months.1:160 group,1:80 group in 9 months of age can be reduced to 1:40 and below,to 12 months of age can be negative.TRUST positive group that TPPA began to decline after TRUST negative,the first March can be reduced by 2 times,then quickly down to 1:20 below.Suggesting that TPPA titer at 1:320 or 3 months of age decreased to 1:320 and below,the latest 9 months of age dropped to a minimum titer of 1:40,can be suggested as syphilis passive antibody.Children born to pregnant women with syphilis that TPPA titer at 1:320 and below,or 3 months of age dropped to 1:320 and below,the latest 9 months of age dropped to a minimum titer of 1:40,can be suggested as syphilis passive antibody.the congenital syphilis children' titer of TPPA was higher,and the titer was maintain higher after treatment,can be maintained for many years in a titer unchanged,3 cases of birth TRUST titer was higher,after treatment,syphilis serum fixed,3 cases TRUST titer lower,6-30 months after treatment turned negative.(5)6 cases of fetal syphilis born with TPPA titer and TRUST titer were higher,but by the treatment of both drops were slow,maybe maintained for many years in a titer unchanged.(6)The overall trend of syphilis passive antibody children' TPPA titers' was the lower the titer,the shorter the median time required.The average seroreversion time of TPPA was boys group longer than in the girls group,and TRUST positive group was longer.The average seroreversion time of TPPA in mothers'TRUST negative group was shorter.Syphilis mother were cured by the treatment,their children' TPPA seroreversion time is the shortest,followed by serum fixation,maintain the original titer group,and the treatment of effective time is longer.Syphilis mothers'treatment time for pregnancy,their children' TPPA seroreversion time shorter than the pregnancy.Conclusion:Pregnant women with syphilis visits throughout all Social groups.Childbearing age syphilis screening is essential,especially before pregnancy,prenatal syphilis screening is an important measure to prevent syphilis mother-to-child transmission.Benzathine penicillin is still the first choice for the treatment of pregnancy syphilis and congenital syphilis.The lower the TPPA initial titer,the shorter the TPPA turns.Children born to pregnant women with syphilis that TPPA titer at 1:320 or 3 months of age dropped to 1:320 and below,such can be suggested as a passive antib6dy to syphilis.Syphilis passive antibodies children'TRUST were negative,mothers' TRUST were negative when during childbirth,treat syphilis before pregnancy and cured were the positive effects of promotes TPPA seroreversion.
