Plastic Treatment For Facial Port Wine Stains

Objective:Port wine stains(PWS)are congenital benign vascular malformations.They are characterized clinically by bored wine-liked lesions and pathologically by ectatic capillaries and postcapillary venules in the papillary and mid-reticular layers of the dermis.Nowadays,laser treatment and surgical treatment are the main therapies for PWS.However,it is difficult to make a proper choice between them.The article is aimed at concluding the contradictions and the key points of treatments.Methods:18 patients diagnosed with PWS from January 2010 to August 2016 were selected in the study.Fabricated flap:Superficial temporal vessels are grafted to certain skin where the expander is to implanted.Then after months,the PWS are removed and the expanded skin are used to repair the lesion.Soft tissue expander:A proper expander is implanted and after months the expanded skin are used to repair the lesion.Skin grafted:The PWS are removed and at the same time,certain skin are grafted to repair the lesion.Results:All patients are satisfied with the surgical treatment.Conclusion:Surgical treatment is a necessary complement to laser treatment and skin grafts,skin soft tissue expanders and fabricated flaps are the three major methods.Skin graft is a simple and convenient way and usually applied in treating large-scaled PWS patients.But the color and texture of the grafted skin is different from surrounding ones.Furthermore,the constraction of grafted skin could lead to serious functional impairment.Compared with skin grafts,skin soft tissue expanders provide soft and similar skin to repair the facial lesions.But without axis vessels and enough pressure,the expanded flaps are not adequate to repair and reconstruct facial tissue and organs,which limit its appliance.Fabricated flaps make up the deficiencies of expanded flaps.The combination of fabricated flap and skin soft tissue expander not only extends the scale of flap but also enhances the efficacy of surgical treatment.Above all,considering the advantages and disadvantages of different treatments,doctors should choose a proper therapy based on the personal conditions of patients.
