ObjectiveThrough collecting the clinical data of schemic Moyamoya disease patients and atherosclerotic cerebral thrombosis patients.Retrospective analysis of the patient's gender,age,serum immune indicators,atherosclerosis indicators and imaging features.To study the risk factors differences between ischemic Moyamoya disease and atherosclerotic cerebral thrombosis,explore the difference between the two pathogenesis;Statistical the distribution of imaging lesions between the two differences,to explore the similarities and differences between their imaging performance.This topic is intended to deepen the understanding of the ischemic moyamoya disease,improve the diagnostic rate,provide a new method to treating the ischemic Moyamoya disease in the future.In order to reduce the mortality and disability of ischemic moyamoya disease,improve the cure rate and create huge social and economic benefits.MethodAccording to the 2014 international diagnostic criteria for cerebrovascular disease to collecting the diagnosed acute(<3day)ischemic stroke patients,cduring the period of 2014-2017,at the First Affiliated Hospital of Nanchang University.selecting patients under 55 years old without cardiogenic embolism,venereal disease,infection,tumor,and severe heart,lung,liver,kidney diseases.According to their head imaging(DSA?CTA?MRA)divided into two groups.Each randomly selected 30 people divided into Case group and control group,The smoking history,blood glucose,blood lipid,liver and kidney function,erythrocyte sedimentation rate,antinuclear antibody,rheumatoid factor,thyroid function(FT3 ? FT4 ? TSH)and antibody(Anti-TG ?Anti-TPO)were examined.Analysising the distribution of schemic lesions,Statistical the differences of the two group patients with atherosclerosis indicators and immune indicators,at last come to the conclusion.ResultsAmong the 30 patients in Case group,14 cases were male and 16 cases were female(mean age(45.72 ± 4.57)years,27 cases were involved in the cerebral cortex,4cases in the basal ganglia,1 cases in the brainstem and cerebellum,4 cases at the Ventricle edge,no one in the thalamus,(6 cases of patients involved in bilateral hemisphere lesions).3 cases had a history of smoking,6 cases had a history of hypertension,3 cases had a history of diabetes,9 cases had hyperlipidemia,and 6cases had antinuclear antibody positive.2 cases had a history of hyperthyroidism,1case of thyroid function(FT3)exceeding the standard,2 cases had thyroid antibody positive(Anti-TG),3 cases ESR exced,5 cases of rheumatoid immunization positive.Among the 30 patients in control group,22 cases were male and 8 cases were female(mean age(50.23± 3.46)years,5 cases were involved in the cerebral cortex,7cases at the Ventricle edge,3 cases in the thalamus,7 cases in the basal ganglia,9 cases in the brainstem and cerebellum,(only one cases of patients involved in bilateral hemisphere lesions).13 cases had a history of smoking,24 cases had a history of hypertension,4 cases had a history of diabetes,7 cases of fasting blood glucose over the standard,15 cases had hyperlipidemia,3 cases had antinuclear antibody positive.one case had thyroid antibody(Anti-TG)positive,one cases ESR exced,4cases of rheumatoid immunization positive.Hypertension,smoking,diabetes mellitus,blood lipid level,FT3,FT4 level and CRP positive rate were significantly different between the two groups(P<0.05).Erythrocyte sedimentation rate,antinuclear antibody,ASO,anti cyclic citrullinated peptide antibody,RF,TSH and anti thyroid antibody(Anti-TG ?Anti-TPO)P>0.05,so there was no significant difference between the two groups.Conclusion1.The proportion of women with ischemic Moyamoya disease was significantly higher than that of patients with atherosclerotic cerebral thrombosis.2.The average age of ischemic Moyamoya disease patients is lower than that of atherosclerotic cerebral thrombosis patients.3.The distribution of lesion for schemic moyamoya disease is not consistent with the distribution of intracranial large blood,common found in the lobar cortex.4.There are differences between the ischemic Moyamoya disease patients and the atherosclerotic cerebral thrombosis patients in atherosclerotic factors,and some risk factors index for atherosclerosis in atherosclerotic cerebral thrombosis patients were higher.5.There is a difference between ischemic Moyamoya disease patients and atherosclerotic cerebral thrombosis patientsin immune factors,some positive rate of immune antibody in ischemic moyamoya disease patients was higher,suggesting that immune factors may play an important role in the pathogenesis of ischemic Moyamoya disease.
