Objective:The aim of this study is to quantitatively analysis changes of left ventricular systolic function by three-dimensional speckle tracking imaging(3D-STI)in obesity children,and to explore the correlation between them and C-reactive protein(CRP),for helping assess the risk of cardiovascular disease in obese children and providing evidence for their early clinical intervention.Methods:Fifty-five obese children(obese group)and thirty-nine overweight children(overweight group)aged seven to fifteen were contrasted with forty-four age and sex-matched healthy subjects(control group).All participants were measured left ventricular end diastolic diameter(LVEDD),end systolic diameter(LVESD),left ventricular end diastolic volume(LVEDV),end systolic volume(LVESV),left ventricular ejection fraction(LVEF),interventricular septal end diastolic thickness(IVST),left ventricular posterior wall end diastolic thickness(LVPWT)and left ventricular relative wall thickness(RWT)by conventional echocardiography.Left ventricular global radial strain(GRS),longitudinal strain(GLS),circumferential strain(GCS),global twist angle(GTA)and left ventricular mass(LVM)were analyzed by 3D-STI analysis software in Tom-tec workstation.Serum C-reactive protein(CRP)levels were detected by specific protein analysis system.Then we compared the difference of left ventricular systolic function,left ventricular remodeling and CRP in obese,overweight and normal groups,and analyzed the correlation between CRP and them.Result:1)Obese group had significantly greater IVST,LVPWT,RWT then control group(P<0.05).Obese group had significantly greater IVST,LVPWT,RWT then overweight group(P<0.05).2)Compared with control group,obese group had significantly lower GRS,GLS,GCS,GTA and greater LVM(P<0.05).Compared with control group,overweight group had significantly lower GRS,GLS,GCS(P<0.05).3)Obese group had significantly higher serum CRP lever then overweight,control group(P<0.05).Overweight group had significantly higher serum CRP lever then control group(P<0.05).There was a positive correlation between serum CRP and BMI(r =0.59,P<0.05).4)There was a negative correlation between the serum level of CRP and GRS,GLS,GCS(r =-0.41,P<0.05;r=-0.36,P<0.05;r=-0.40,P<0.05).There was a positive correlation between the serum level of CRP and RWT,LVM(r =0.29,P<0.05;r =0.29,P<0.05).Conclusion:1)In the early stage of childhood obesity,the left ventricular systolic function has been impaired.2)The level of serum C reactive protein was significantly increased in the early stage of childhood obesity,and it was closely related to left ventricular dysfunction.3)Three-dimensional speckle tracking technique can be used to quantitatively analyze the of left ventricular myocardium in obesity children,and it is more sensitive and accurate to detect the early changes of myocardial function than the conventional two-dimensional echocardiography.
