Objective: Postoperative infection of knee arthroplasty is the most serious complication of knee arthroplasty.It not only threatens the life of patients,affects their quality of life,but also brings a lot of trouble to clinicians.This study mainly combines case study and analysis of risk factors associated with infection after artificial knee arthroplasty,to prevent postoperative infection,improve the clinical efficacy of patients,and improve the quality of life after operation.Methods: Retrospective analysis of all patients underwent total knee arthroplasty for the first time from June 2013 to September 2016 of our hospital.Follow up by telephone,ask the doctor for treatment,and consult the patient's medical records to determine the infection.To analyze the diagnosis and treatment process and bacterial culture results of patients with infection group,and to find out the probable factors of infection.The main factors of preoperative patients,perioperative period and operative factors and postoperative factors were involved,including sex,age,diagnosis,preoperative albumin amount,preoperative ESR,CRP,surgical history,anesthetic mode,operative site,allogeneic blood transfusion.Investigate and analyse the bacterial culture and drug susceptibility of infected cases in combination with infection cases.The effects of some related factors on postoperative infection of artificial knee arthroplasty were analyzed statistically and discussed.Result:In the study,527 cases of total knee arthroplasty were selected,including 5patients with postoperative infection,and the infection rate was 0.95%.The results of statistical analysis showed that the effect of sex,age,diagnosis,anesthetic mode and operative location on the infection rate of total knee arthroplasty was not statistically significant.Postoperative infection rate(5.56%)was high in patients with puncture or surgical history;The incidence rate of patients with abnormal ESR and CRP in preoperative patients(3.7%)was high.The preoperative albumin was lower than the non-infected group in the infected group.Postoperative patients with the history of blood transfusion infection rate of 3.41%,no blood transfusion patients group infection rate of 0.89%.High rate of postoperative infection rates in patients was with low serum albumin,joint puncture or surgical history,ESR,CRP abnormalities before operation and blood transfusion history after operation;Bacterial culture of infected group was the result of Gram positive bacteria accounted for 54.5%,Gram negative bacteria accounted for 27.3%,fungi accounted for 18.2%,and the drug resistance rate of pathogens was higher.Conclusion: The patients with low serum albumin,joint puncture or surgical history,preoperative patient ESR,CRP abnormalities,postoperative patients with blood transfusion history had the risk factors of postoperative infection after knee arthroplasty;After knee arthroplasty,infected gram positive bacterium can be seen,and can be combined with fungal infections.
