Objective: To estimate the incidence of triple negative breast cancer(TNBC)in all of breast cancer patients,to explore the clinicopathologic characteristics,the transfer mode of recurrence and survival situation of TNBC,and to analyze the correlation factors affecting the prognosis of TNBC,which will provide clinical reference for clinicians making individualized treatment plan of TNBC.Methods: Retrospectively analyzed the the clinicopathological characteristics and prognosis of the 378 primary breast cancer patients with complete clinical pathology and follow-up data in First affiliated hospital of bengbu medical college between January 2010 and December 2011.According to the testing results of immunohistochemical staining of estrogen of receptor(ER),progesterone receptor(PR)and human epidermal growth factor receptor 2(HER-2)into TNBC group(61 cases)and the non-triple negative breast cancer group(non-TNBC,317 cases),in order to compare two groups in the differences between clinicopathological characteristics and prognosis of survival.Results: This study collected 61 cases of patients in TNBC group,the incidence of TNBC that accounted for all breast cancer patients in the same period was 16.14%(61/378).Compared with non-TNBC group,the patients in TNBC group had significant differences in the following aspects: the age of patients was less than 35 years old when been confirmed,patients were with family history of breast cancer,the maximum diameter of tumor was more than 5 cm,the preoperative axillary lymph node status was positive,clinical staging of tumor was stage ?,histological grade of tumor was level III,Ki-67 was overexpression and P53 was overexpression(P< 0.05),while there is no statistical difference in the aspects of menstrual status,pathological type and surgical method between them.The local recurrence and distant metastasis rate in TNBC group was higher obviously than in non-TNBC group(9.84% vs 2.52%,P=0.015<0.05;21.31% vs 11.04%,P=0.035<0.05),and local recurrence was given priority to with chest wall recurrence,distant metastasis had obvious tendency of human organs,common in lung,brain,liver metastasis.5-year disease-free survival(DFS)and5-year overall survival(OS)in TNBC group were significantly lower than in non-TNBC group(68.85% vs 88.96%,P=0.045<0.05;88.52% vs 95.58%,P=0.001<0.05).The study found the factors that was thought to be associated 5-years DFS by univariate analysis in TNBC group is: age,the maximum diameter of tumor,the preoperative axillary lymph node status,clinical staging of tumor,P53 overexpression;While the maximum diameter of tumor,the preoperative axillary lymph node status and clinical staging of tumor the factors were recognized as the influence factors of 5-years OS.Multivariate Cox regression analysis found that independent factors affecting5-years DFS in TNBC group is: age,the maximum diameter of tumor,the preoperative axillary lymph node status;While the maximum diameter of tumor,and the preoperative axillary lymph node status were the independent factors influencing 5-years OS.Conclusion: The incidence of TNBC accounted for 16.14% of all breast cancer patients in the same period.Compared with non-TNBC,the clinicopathological characteristics of TNBC include: younger onset age,with breast cancer family gathered phenomenon,larger the maximum diameter of tumor,positive the preoperative axillary lymph node status,later clinical staging of tumor,higher histological grade of tumor,easier to local recurrence and distant metastasis,lower 5-years DFS and 5-years OS,which suggests that high degree malignant led to poor overall prognosis for TNBC.The recurrence and transfer mode of TNBC is unique,and distant metastasis had obvious tendency of human organs,common in lung,brain,liver metastasis.The factors of age,the maximum diameter of tumor,the preoperative axillary lymph node status,clinical staging of tumor,P53 overexpression especially the maximum diameter of tumor and the preoperative axillary lymph node status play the important clinical roles in judging the prognosis of TNBC.
