Objective: To provide scientific evidence for formulating the future prevention and control strategy of schistosomiasis and optimizing resource allocation.Schistosomiasis prevention and control effect,factors and epidemics were analyzed and evaluated by collecting relative data.Method: Implementation of prevention and control strategies of schistosomiasis in Dali Couty from 2004 to 2015 from different aspects,such as health,agriculture,forestry and water conservancy was retrospectively collected.Number of successful strategies successfully implementated and cost of schistosomiasis prevention and control and changes of endemic situation of schistosomiasis in human,animal and Oncomelania hupesis snails were surveyed.Effects and factors of prevention and control were analyzed.Epidemiology situation of schistosomiasis in every town of Dali County in 2015 was evaluated by using descriptive analysis,cost-effectiveness analysis,multiple linear regression,TOPSIS and rank sum ratio.Result:(1)In 2015,snail area was 17.64hm~2 and reduction ratio was 93.94%.The occurrence rate of frames with snails was 0.26% and reduction ratio was 88.37%.The density of living snail was 0.1 snails/0.1m~2 and reduction ratio was 93.63%.Serological positive rate of human was 2.00% and reduction ratio was 73.22%.The adjusted infection rate of human was 0% and reduction ratio was 100%.The infection rate of cattle was 0% and reduction ratio was 100%.(2)Based on epidemiological data in 2004,multiple effectiveness index were 44.10,47.39,74.47,81.88,82.36,83.12,83.67,84.00,84.70,84.89,84.99 in each year from 2005-2015.(3)Based on Epidemic data in 2004,as the object of assessment the year of 2008 and 2015,the average annual cost of infection rate reduction of 1% per 100 persons mean were ?21and ?20 respectively.The average annual cost of infection rate reduction of 1% per100 cattle were ?486 and ?502 respectively.The average annual cost of snail area reduction of 1% per 1hm~2 were ?773 and ?1269 respectively.The average annual cost of density of living snail reduction of 1% per 1hm~2 were ?725 and ?1362 respectively.The average annual cost of infection rate reduction of snails of 1% per 1hm~2 were?634 and ?1192 respectively.The average annual cost of multiple effectiveness index increment of 0ne were ?135600 and ?129300 respectively.(4)Serological positive rate of human was significantly associated with infection rate of cattle and popularizing rate of harmless sanitary household latrine.Adjusted infection rate of human was significantly associated with rural safe drinking water penetration rate.Infection rate of cattle was significantly associated with popularity rate of harmless toilets and serological positive rate of human.Infection rate of snails was significantly associated with rural safe drinking water penetration rate,infection rate of cattle,coverage rate of chemotherapy of human and popularity rate of harmless toilets.(5)Among 10 towns with schistosomiasis,the mildest town was Taiyi,while the most severe was Yinqiao by using TOPSIS.Regression equation of schistosomiasis epidemic situation was Cipro=0.120Probit-0.383(F=105.712,P<0.05,R2=0.991)by using rank sum ratio.Based on schistosomiasis epidemic situation,the severe towns were Yinqiao,the middle towns were Wanqiao,Shangguan,Xiaguan,Haidong,Dali,Shuanglang,Xizhou and the mild towns were Fengyi and Taiyi.Conclussion: Schistosomiasis prevention and control effect is significant after prevention and control strategy of schistosomiasis with emphasis on infectious source control.Schistosomiasis epidemic was greatly curbed.Although the epidemic is low popular in general at present,the cost was relatively high.The main measures affecting s chistosomiasis prevention and control were construction of harmless toilets,safe drinking water project and chemotherapy of human.At present,schistosomiasis epidemic was not same in different town.In some towns,schistosomiasis epidemic was unsteady,and the task of prevention of schistosomiasis was arduous.
