In The Application Of Anal Fistula Balloon Ileum In Low Rectal Cancer

Objective:To investigate the application value of prophylactic terminal ileum adjustable ileal fistula in patients with rectal cancer underwent resection of abdominal rectal cancer(Dxion).Methods: Review of Wangnan Medical College affiliated Yijishan Hospital of gastrointestinal surgery from April 2015 to February 2017 were parallel transabdominal radical resection of low rectal cancer(Dxion)analysis of 41 cases of low rectal cancer cases.Of which 20 cases from April 2015 to March 2016 were treated with traditional ileostomy(traditional fistula group),and stoma closure was performed about half a year after operation;In another group,21 cases of patients with low rectal cancer from the terminal ileum 10-15 cm in ileum proximal ileal fistula in a balloon,modified from the No.7.5 reinforced tracheal catheter(fistula group).After 28 th days to confirm the abdominal wall fistula was identified,can to remove the catheter.With vaseline gauze filling leakage,cover gauze and abdominal compression fixation,after 24 hours treatment,remove the Vaseline gauze,the fistula would be healed.Results: The incidence of anastomotic leakage in two groups was 0%(0/33)vs 5%(1/20),Other postoperative complications was 23.8%(5/21)vs.30%(6/20),VAS was(6.9±1.0)vs.(7.3±1.1).The difference was not statistically significant(all P>0.05).While the fistula group in hospitalization time,(14.4 + 2.4)than(17.2 + 2.2)days,the treatment fee(3.9 + 0.4)than(4.8 + 0.7)million yuan,the exhaust time of(3.8 + 1)than(4.5 + 1.1)days and eating time of(4.2 + 1)than(5.8 + 1)days,were less than the traditional colostomy group.The difference was statistically significant(all P<0.05).Conclusion: For high-risk patients may anastomotic fistula after operation,the abdominal radical resection of low rectal cancer(Dxion),for the terminal ileum catheter,simple and practical,can effectively prevent the occurrence of anastomotic fistula,shorten the hospitalization time and treatment,reduce the economic burden,also can avoid the two reduction operation.To alleviate the suffering of patients.
