Objective:The serum was measured at different time points after injury in patients with TBI thyroid stimulating hormone(TSH),growth hormone(GH),follicle stimulating hormone(FSH),prolactin(PRL),adrenocorticotropic hormone(ACTH)content,to investigate the relationship between changes of hormones related to the anterior lobe and the severity and prognosis of patients. To provide more theoretical support for the clinical diagnosis and related hormone replacement therapy in patients with TBI before and after injury. Methods:From Wangnan Medical College affiliated yijishan Hospital Department of neurosurgery in October 2015 to October 2016 during the period of the hospitalized patients with TBI were collected, and according to the inclusion criteria and exclusion criteria, select one of the 90 eligible cases, according to patients, the severity of the disease according to Glasgow coma(Glasgow coma standard for evaluation of scale,GCS)grouping: reference the GCS score,13 to 15 points as mild,a total of 32 cases,9 to 12 points as moderate,a total of 38 cases,6 to 8 points as severe,a total of 12 cases,3 to 5 points for the severe,a total of 8 cases;through its first days after the injury, were measured in third days. The content of serum for seventh days and fourteenth days in GH,TSH,FSH,PRL and ACTH,changes in hormone levels in the dynamic monitoring of TBI patients after injury at different time points, and analyze the relationship between prognosis between patients and after. In addition,40 healthy persons who had no history of endocrine system diseases were selected as the control group. Results:The study of all TBI patients after injury at different time points(first,3,7,14 days)of each hormone content detection, the highest incidence of GH,FSH and ACTH anomalies, while TSH and PRL are abnormal in the injury rate is relatively low;GH,TSH,ACTH,FSH,PRL content were increased after injury, mainly three days ago, especially GH,ACTH and FSH,the change is more obvious, first and 3 days were significantly higher than the control group, the difference was statistically significant, one week after all hormones were gradually restored;mild and moderate patients after injury various hormone were increased, but the overall modest, severe and hormone content in injured patients with TBI growth rate, much higher than the control group, the difference was statistically significant;Compared group hormone content and prognosis of patients with poor prognosis were the poor prognosis of patients with GH was statistically significant compared with good content and prognosis of first and 2 days, there was statistical significance in TSH and PRL first days after the injury only in content and good prognosis group and good group content and prognosis of ACTH after injury first,3,7 days compared with statistical significance, and good content and prognosis of FSH after injury first,3,7,14,there were statistically significant differences.Conclusion:(1)related hormone levels in patients with TBI after injury in the anterior pituitary secretion is abnormal, the most common is GH,FSH and ACTH anomalies;(2)TBI patients with hormone abnormalities after injury mainly in the first three days, the most obvious change of the content of each hormone recovered(3);changes of hormone content of mild TBI patients is generally not very obvious,no clinical significance, severe and TBI changes of patients with hormone content,is closely related with the severity of disease itself, has important clinical significance;(4)TBI patients after injury hormone content is of clinical value of change evaluation is very important the prognosis for patients,first,3 days after injury, abnormal, obvious hormone surges and sags, suggests the prognosis is not ideal,on the contrary, modest changes in hormone content, showed a relatively stable state, multi prognosis good.
