Objective:To explore the change and clinical significance of serum Aquaporin-4 and phosphorylated axonal form of the high molecular weight neurofilament subunit NF-H in traumatic brain injury(TBI) patients.Method: A prospective control study was carried out. A total of 31 patients with mild,moderate and severe TBI(the observation group) and 20 healthy controls(the control group) were enrolled in this study.Serum samples were collected within 24 h, 48 h, 72 h and120h post injury.Serum AQP4 and p NF-H were estimated using ELISA to analyzed the change trend of them together with Glasgow Outcome Scale.Result:1. Compared with control groups,the serum AQP4 in patient with mild TBI presented no significant difference in all the time(P>0.05).Serum levels of AQP4 in patients with moderate and severe groups were higher than those in the control group in all the time(P<0.05). The serum AQP4 in patients with severe TBI were higher than those in patients with moderate TBI.The serum levels of AQP4 were significantly higher in patients with poorer prognoses compared with those with better prognoses(P<0.05).2. Serum levels of pNF-H in the observation group wer significantly higher than those in the control group in all the time(P<0.05). Serum levels of p NF-H in patients with moderate group was higher than the mild group from 24 h to 48 h,but showed no significant difference from 72 h to 120 h.The serum p NF-H in patients with severe TBI were higher than those in patients with moderate TBI.The serum levels of p NF-H were significantly higher in patients with poorer prognoses compared with those with better rognoses(P<0.05).3.Serum levels of AQP4 and pNF-H in the observation group at 24 h after TBI presented negative correlation with GOS( AQP4:r=-0.733, P<0.05; p NF-H:r=-0.595, P<0.05).Receiver operating characteristic curves(ROC) analysis showed the AUC of AQP4(0.930,p=0.001) and p NF-H(0.808,p=0.001).Conclusion:Determination of serum AQP4 and p NF-H in TBI during early period may be useful to evaluate pathogenic condition and prognosis.
